Requirements
- A Bachelor's degree in Pharmacy and less than one year of relevant ambulatory or community pharmacy experience is required, or an equivalent combination of education and experience.
- Successful completion of an accredited Post Graduate Year 1 pharmacy practice program is required.
- A Pharmacist license in New York State is required upon hire.
Responsibilities
- Dispense medications for patients seen in an outpatient pharmacy.
- Oversee and directly contribute to compounding, preparation, and dispensing of medications in accordance with applicable state and federal pharmacy laws and regulations.
- Answer medication-related questions from patients and providers and provide counseling on proper medication use.
- Supervise pharmacy technicians and direct their work to ensure appropriate workflow.
- Understand and follow departmental and hospital policies and procedures related to medication dispensing.
- Dispense medications accurately and in a timely manner.
- Resolve problem prescriptions independently and appropriately.
- Process and validate prescriptions using current information-system technology to create and maintain accurate patient profiles.
- Check products requiring pharmacist approval before dispensing.
- Participate in departmental quality-assurance processes, including reporting and following up on medication incidents, reconciling medication administration records, and reporting adverse medication events and errors.
- Develop or participate in clinical practice by interacting with physicians, nurses, other healthcare professionals, and patients as a member of an interdisciplinary team to guide and recommend appropriate medication use.
- Participate in focused clinical initiatives, including immunization, discharge planning, transplantation, wellness, and screenings.
- Monitor assigned patient populations for clinical progress, including renal and hepatic function, white blood cell counts, and other applicable laboratory tests.
- Recommend therapy adjustments as necessary.
- Document clinical activities and interventions regularly.
- Participate in educating Pharmacy staff and other healthcare professionals.
- Identify the need for patient teaching and perform patient-teaching tasks when necessary.
- Resolve problems through clear and direct communication with pertinent staff.
- Proactively identify patient-care problems, make reasonable attempts to resolve them, and communicate progress between shifts.
- Identify and resolve barriers to patient discharge, including financial barriers, when practicing in a hospital setting.
- Solve problems requiring interdisciplinary-team participation or revision of hospital or departmental policies, procedures, or treatment guidelines.
- Perform charge-pharmacist duties when scheduled, including coordinating shift resources, resolving staff conflicts, and communicating appropriately between shifts.
- Contact a supervisor when a solution is beyond the pharmacist's expertise or authority.
- Perform other duties as assigned.
