Requirements
  • Current licensed vocational nurse or licensed practical nurse licensure in good standing in the state or states of practice, with one year of experience under the license.
  • Ability to collaborate daily with nurses, social workers, chaplains, volunteers, and certified nursing assistant leadership.
  • Ability to work independently while handling phone, email, written communication, meetings with healthcare professionals, and hospice rules and regulations.
  • Time-management ability to manage visits, documentation, meetings, and urgent needs by prioritizing and triaging multiple concerns.
  • Ability to learn an electronic medical records system.
  • Willingness to drive with reliable transportation, a valid driver's license, and auto insurance to travel across the service area.
Responsibilities
  • Provide care and services for a group of hospice patients.
  • Visit patients where they reside, including private homes, assisted living facilities, and hospitals.
  • Work after-hours on-call for seven days or nights on followed by seven days or nights off.
  • Work closely with patients on a one-to-one level while receiving support from the patient care team.
