Requirements
  • Current registration as a Licensed Practical Nurse in the applicable province or state.
  • Experience working in a seniors' environment and with unregulated care providers is preferred.
  • Education and experience in gerontology, rehabilitation, community care, complex care, or long-term care are an asset.
  • Strong written and oral communication, critical thinking, care coordination, time management, and customer focus skills are required.
  • Demonstrated sound clinical knowledge is required.
  • Current Basic Cardiac Life Support and First Aid certification are required.
  • Computer proficiency and experience with Microsoft Office applications are required.
  • A complete and current Vulnerable Sector Check, including a Criminal Background Check, must be provided within six months or obtained.
Responsibilities
  • Work as a member of the inter-professional team using the person-centered approach to care.
  • Complete and ensure residents' clinical care assessments, care/service plans, and documentation are accurate and up to date, and communicate changes as required.
  • Implement and evaluate the effectiveness of care/service plans and make changes as required.
  • Assist with scheduling residents' care conferences and participate in conferences where applicable.
  • Work closely with the Director of Care to facilitate residents' move-ins, move-outs, and transfers.
  • Provide direct resident care and medication and treatment administration, and document care in compliance with regulatory and resident-care standards, policies, and procedures.
  • Process orders, safeguard drugs and narcotics, and administer them in compliance with regulatory requirements, scope of practice, policies, and procedures.
  • Facilitate and provide resident and family education.
  • Plan, coordinate, manage, and evaluate residents' care plans with residents or substitute decision-makers, physicians, nursing and interdisciplinary teams, and contracted service providers.
  • Coordinate nursing-team resident-care assignments to ensure efficient and timely delivery of services to residents.
  • Respond to and manage significant changes in residents' conditions and medical emergencies, and provide emergency first aid.
  • Teach, delegate to, and direct unregulated care providers.
  • Coordinate emergency response in the absence of the Director of Care or Director of Health and Wellness or Executive Director.
  • Respond to resident and family concerns, ensure appropriate action is taken, and communicate concerns to the Director of Care or Director of Health and Wellness or Executive Director and other departments.
  • Follow and assist with the coordination, implementation, and monitoring of Infection Prevention and Control, Health and Safety, Clinical Programs, and Quality and Safety Plans for the Nursing Department.
  • Work in a position of trust with vulnerable adults.
  • Uphold and promote the organization's values and philosophy relating to ethics, morality, and integrity as set out in the Code of Conduct.
  • Complete other duties as assigned.
