- Provide psychosocial assessments, crisis intervention services, and counseling to patients.
- Coordinate healthcare services throughout the continuum of care on behalf of patients and assist with discharge planning.
- Conduct thorough psychosocial assessments of patients, including emotional, cultural, and financial needs, using all available resources.
- Assist with evaluation and referral in situations involving possible abuse or neglect for adult and pediatric patients.
- Implement social-service interventions based on psychosocial assessments.
- Counsel patients and families regarding adjustment to illness, treatment, grieving, and conflict resolution.
- Assist patients and family members in obtaining advance directives and durable power of attorney for healthcare.
- Collaborate with case managers and the healthcare team to formulate and facilitate hospital discharge plans.
- Act as a liaison among patients, family members, physicians, interdisciplinary teams, external agencies, and organizations, making referrals as appropriate.
- Facilitate meetings to discuss plans of care and discharge planning.
- Maintain and submit required documentation in a timely manner according to agency policies and procedures.
- Dictate psychosocial histories and initiate development of treatment plans and psychosocial clinical protocols where applicable.
- Coordinate and provide ongoing support programs to patients and families where applicable.
