Requirements
  • Registration in good standing with the College of Nurses of Ontario.
  • An RPN diploma or RN diploma/degree from a recognized college or university.
  • A valid First Aid or CPR certification.
  • Effective written and verbal communication and documentation skills.
  • Reliable attendance, punctuality, and availability for the role.
  • Commitment to lifelong learning and professional growth, with the ability to apply critical thinking to provide client care.
  • A valid Ontario driver's license (G license) and a reliable vehicle for travel.
  • Morning and day availability and participation in a bi-weekly weekend rotation from 8:00 a.m. to 4:00 p.m. or 9:00 a.m. to 5:00 p.m.
Responsibilities
  • Assess, monitor, and make care plans and care coordination arrangements.
  • Evaluate care plan interventions through ongoing reassessment and evaluation of results.
  • Ensure quality patient care by identifying and acting on factors that affect practice autonomy and delivery of quality care.
  • Demonstrate professionalism, leadership, judgment, and accountability in interactions with clients and families.
  • Support safe client care through developing knowledge and skills in accordance with continuous quality improvement principles.
  • Inform the nursing supervisor of client activities, relevant concerns, or changes in the client's condition or care plan, and submit appropriate documentation to the case coordinator.
  • Maintain documentation standards according to the College of Nurses of Ontario.
  • Provide and coordinate home health nursing services to individuals, families, and groups in homes, schools, retirement homes, and other community settings.
Desired Qualifications
  • Membership in a professional association such as RNAO or WeRPN.
  • Moderate wound-care or venipuncture knowledge and experience.
  • Acute-care and medical-surgical nursing experience.
  • Knowledge of community nursing.
