- An active independent clinical license such as LCSW, LMFT, LPC, or an equivalent license with the ability to practice independently.

- Fulfill foundational Behavioral Health Care Manager responsibilities within the Collaborative Care Model program in accordance with University of Washington AIMS Center guidelines.
- Support the mental and physical health care of assigned patients by coordinating with medical providers and other mental health professionals.
- Screen and assess patients for mental health disorders and facilitate engagement and follow-up care.
- Educate patients and caregivers about mental health disorders and available treatment options.
- Monitor clinical symptoms and treatment outcomes, document progress, and adapt care plans under supervision as needed.
- Deliver evidence-based behavioral interventions, including motivational interviewing, behavioral activation, and problem-solving treatments.
- Participate in case reviews with psychiatric consultants and licensed neuropsychologists to support treatment plans for patients who are not improving as expected.
- Facilitate referrals to external services, including social services, vocational rehabilitation, and specialty care, as clinically indicated.
- Maintain accurate records in the electronic health record system and ensure clear communication with care team members.
- Assist in developing and implementing relapse prevention plans for patients who achieve treatment goals.
- Build relationships with patients, caregivers, and partners to foster trust and ensure continuity of care.
- Background or clinical interest in ALS, Parkinson’s disease, dementia care, or other neurodegenerative or chronic disease care.
- Familiarity with insurance processes and medical billing and coding.
- Experience reviewing medical records and exposure to clinical trials.
- Ability to adapt and thrive in a startup or rapidly growing organization.
- Knowledge of current trends in neurodegenerative diseases, terminal illnesses, targeted therapies, and clinical trials.

Synapticure offers specialized evaluations, genetic counseling, and more The virtual care company Synapticure has launched a new program to offer remote neurological care for people with Friedreich's ataxia and other types of ataxia. According to a press release from Synapticure, the new program can in some cases serve as a patient's primary neurologist, or it can partner with existing care teams. Among other services, the program offers specialized evaluations, genetic counseling, comprehensive longitudinal care, and education about clinical trial opportunities, the company said. The new program will be led by Elizabeth Ferluga, MD, director of movement disorders at Synapticure. "Patients with ataxias are a community that has been underserved by the traditional healthcare system for far too long," Ferluga said. "The Synapticure model is transformative for this population - we can meet patients where they are, perform a thorough evaluation, help them understand their diagnosis and what it means for them and their families, provide information about potential clinical trials, and be a consistent, knowledgeable partner in their care over time. Ataxia is a type of movement disorder characterized by a lack of balance and coordination. Friedreich's ataxia is one specific form of ataxia that is caused by mutations in the FXN gene. There are also many other forms of ataxia. Collectively, these conditions affect tens of thousands of people in the U.S. alone. People with ataxia typically rely on expert neurologists in order to receive optimal care. But actually finding neurologists with expertise in ataxia can be a major struggle, especially for people who don't live close to academic specialty centers. By offering a virtual option, Synapticure aims to make this type of specialized treatment easier to access. "For people living with ataxias and their families, accessing expert neurological care is often a years-long struggle," said Sandra Abrevaya, CEO and co-founder of Synapticure. The virtual-first care model connects patients to expert neurologists, genetic counselling, and comprehensive longitudinal care delivered through Synapticure's independent healthcare providers.
