Requirements
  • An active and unrestricted Registered Nurse license is required.
  • A minimum of 2 years of experience in the healthcare industry is required.
  • Experience establishing and maintaining relationships with individuals at all levels of the organization, in the business community, and with vendors.
  • Experience applying knowledge of standard practices for all services offered and current relevant and applicable standards, including ACHC and URAC standards.
  • Experience providing customer service to internal and external customers, including meeting service quality standards and evaluating customer satisfaction.
  • Basic personal computer skills, including competent use of the internet, email, Microsoft Word, Microsoft Excel, and Microsoft PowerPoint.
  • Experience identifying operational issues and recommending and implementing strategies to resolve issues and improve processes.
  • Access to reliable transportation for travel to care facilities, home visits, and multiple hospitals; a valid driver's license and proof of insurance are required when using a personal vehicle.
  • Willingness to obtain nursing licensure in additional states when business needs and geographic alignment support it.
  • Ability to plan, organize, and make presentations.
Responsibilities
  • Evaluate, educate, and train patients, caregivers, and facility staff on how Option Care Health services and products will be facilitated at alternative sites, in homes, or virtually to ensure successful patient transitions to the Option Care Health delivery model.
  • Conduct patient assessments and evaluations to determine patient viability for the Option Care Health care delivery model.
  • Partner and coordinate with the Option Care Health sales team to understand clinical transition education needs and develop improvements aimed at increasing patient transition volume.
  • Maintain confidentiality of patient and proprietary information and observe legal guidelines for safeguarding that information.
  • Document education properly using Option Care Health applications and technology.
  • Coordinate patient transitions from hospitals to homes or alternate care settings by collaborating with facility staff, providers, and internal teams to ensure timely discharge, therapy education, and continuity of care.
  • Provide in-person patient and caregiver education, gather pre-admission and insurance information, and facilitate initiation of services to support safe and effective care at home.
  • Travel 100% locally to partner facilities, community hospitals, and medical practice offices to sell Option Care Health services, process referrals, and provide live education and training support to patients, caregivers, and referral sources.

Option Care Health reported a 13% revenue increase in Q4 2025, with acute therapies growing in the mid-teens and chronic therapies in the low double digits.
