Requirements
- A Master’s Degree in Social Work or equivalent.
- One year of clinical and/or medical social work experience.
Responsibilities
- Maximize the social functioning of patients and families impacted by illness by providing appropriate social service intervention as part of a multidisciplinary team.
- Apply a strengths-based and person-centered philosophy of care and demonstrate practical application of social work core values.
- Provide direct social work service and counseling to patients and their families, and float to other areas when needed.
- Gather appropriate data and formulate relevant assessments of patients’ and families’ medical, social, and emotional situations using effective interviewing techniques, consultation with the healthcare team, and chart review.
- Assess and identify the social determinants of health of patients.
- Provide social work intervention and support for patients’ and families’ complex social and emotional needs, including individual and family counseling, crisis intervention, education, and linkage with resources for patients, families, and staff as indicated by the assessment and treatment plan.
- Identify and understand the behavioral and emotional factors underlying a patient’s illness.
- Assess and support care coordination that meets individualized patient needs.
- Provide crisis intervention and assist families in understanding the implications and complexities of the medical situation and its impact on lifestyle.
- Assist with discharge planning and post-acute placement for complex discharges.
- Accurately document assessments, plans, interventions, patient and family involvement, outside agency communications, interdisciplinary contacts, and case closure according to social work and patient care area standards.
- Work flexibly on other hospital floors or teams as needed by management.
- Participate actively in department activities, including regular staff meetings, committees, and task forces.
- Provide orientation, work direction, and guidance to less experienced social workers.
- Perform other duties as assigned.
Desired Qualifications
- A current California Licensed Clinical Social Worker license.
- Previous experience as a social worker in an acute medical setting.
- Knowledge of contemporary behavioral and systems theories relevant to healthcare, end-of-life dynamics and interventions, grief and bereavement counseling, substance abuse identification and interventions, and support for victims of abuse, neglect, or violence.
- Knowledge of community or financial resources for underserved or vulnerable populations.
- Bilingual proficiency in English and Spanish.
