Requirements
  • An associate’s degree in Human Services or a related field of study.
  • Three years of work experience in a human services position.
  • A valid New York State driver’s license and reliable transportation.
  • Ability to utilize a personal vehicle to conduct business when necessary.
Responsibilities
  • Conduct an organized central intake process, including prompt applicant screening and appropriate placement while maintaining expected utilization rate goals across agency programs.
  • Regularly assess the processes and tools used to maintain an active and updated intake waitlist and placement.
  • Complete Intake Assessments with prospective residents, identify barriers to placement, and work to address those barriers.
  • Complete Level of Care for Alcohol and Drug Treatment Referral screenings to determine appropriate levels or elements of care.
  • Ensure appropriate resident placement in agency programs while prioritizing resident needs and expected utilization rates.
  • Integrate Trauma-Informed Care and its five principles into duties and interactions with staff, residents, and external partners.
  • Develop and maintain networking trainings with outside providers regarding agency services and referral processes.
  • Work with staff and referral sources to complete pre-admission financial screening, social service, and third-party payee linkages.
  • Assist applicants and residents with paperwork for third-party reimbursements, ensuring documents needed to qualify for Congregate Care Level II Social Services, Supplemental Security Income, Social Security Disability, or other third-party payee programs are obtained.
  • Facilitate completion of sliding fee forms for self-payers.
  • Ensure prospective residents have Department of Social Services approval.
  • Maintain positive relationships with parties responsible for payments to the agency.
  • Collaborate with Finance and Clinical Services teams to review financial and clinical needs related to intake.
  • Input required financial and intake information into the agency database.
  • Ensure prospective residents have an active in-network Managed Care Organization or assist applicants who need Medicaid managed care with information and direction.
  • Maintain compliance with Federal Confidentiality requirements under 42 CFR Part 2, HIPAA, Medicaid, HUD, and other applicable rules and regulations.
  • Maintain regular and consistent attendance at the work location.
  • Complete other duties as assigned.
