Requirements
  • A minimum of a bachelor's degree in human services or a related field is required.
  • Two years of experience working with youth or families.
  • FCT Certification is required and must be completed within one year of employment to continue providing FCT services and maintain employment.
  • Mississippi candidates must be provisionally credentialed as a PCSS in Mississippi with eligibility to be fully credentialed as a CCSS in Mississippi.
  • North Carolina candidates must meet qualifications as a Qualified Professional (QP) with experience serving children and adolescents with a primary mental health diagnosis.
  • Ohio candidates must have an LSW or eligibility for provisional licensure preferred.
  • Virginia candidates must have a current Qualified Mental Health Professional (QMHP) credential or the ability to register as a QMHP with the Virginia Board of Counseling.
  • Employment is contingent upon meeting the requirements of a comprehensive background investigation prior to joining the team.
Responsibilities
  • Provide Family Centered Treatment services according to the service definition and model fidelity requirements.
  • Complete FCT Checkoffs and other internal trainings within designated deadlines through the assigned e-learning system.
  • Deliver FCT services to children, adolescents, and families primarily in their living environments with a family focus.
  • Provide 24/7 on-call coverage to a caseload of families and children, including service provision in homes and communities according to family convenience.
  • Complete required clinical records and FCT documents with children and families and enter or upload them timely in the Electronic Health Record according to company standards.
  • Develop the Service Plan in coordination with the Treatment Team Process and maintain it according to designated authorization processes.
  • Work collaboratively with identified Treatment Team members to ensure effective communication and coordinated service provision.
  • Conduct ongoing revisions of the Service Plan and implement the Service Plan.
  • Provide psychoeducation as indicated in the Service Plan.
  • Conduct family interventions aligned with the FCT model, facilitate movement through the four phases of treatment, and complete all fidelity documents.
  • Assist families and children in establishing weekly goals and identifying steps to achieve those goals.
  • Make referrals to services and resources on behalf of families and children.
  • Develop and coordinate discharge plans based on treatment, including relevant aftercare needs and plans.
  • Access community resources beneficial to clients' aftercare needs.
  • Coordinate and oversee initial and ongoing assessment activities.
  • Convene the Treatment Team for the Service Plan.
  • Consult with medical and non-medical providers, engage community and natural supports, and include their input in person-centered planning.
  • Ensure linkage to clinically appropriate and effective services, including arranging psychological and psychiatric evaluations.
  • Provide and coordinate behavioral health services and other interventions for youth or family members with behavioral health professionals and Treatment Team members.
  • Monitor and document consumer progress.
  • Participate in Quality Assurance and Quality Improvement activities, including audits, corrections, and designated improvement plans and projects.
