Requirements
  • Minimum of one year of experience as a Registered Nurse in an acute care setting.
  • A current Registered Nurse license in the state of employment.
  • A valid driver's license and auto insurance with the applicant's name listed as a driver.
  • Ability to communicate effectively with patients, families, physicians, and co-workers.
  • Knowledge of nursing best practices and ability to make appropriate nursing judgments.
  • Ability to identify situations and determine appropriate solutions.
  • Ability to prioritize work, meet deadlines, follow directions, and work as a team member.
  • Ability to read, speak, and write English.
  • Ability to use computers and personal digital assistants.
  • Ability to meet the physical demands of the role, including walking, bending, standing, sitting, driving, lifting up to 50 pounds, reaching, climbing, stooping, and maintaining hand and finger dexterity.
  • Ability to distinguish smell and temperature and perform required hearing, vision, and in-person and telephone communication activities.
Responsibilities
  • Oversee the overall care of each patient from the beginning to the end of the episode.
  • Update physician orders in the patient chart and notify the physician of significant changes in the patient's condition.
  • Manage multidisciplinary care as applicable.
  • Provide skilled nursing care in patients' homes as ordered by the attending physician.
  • Observe and monitor patient conditions.
  • Perform OASIS assessments to develop individualized plans of care and adjust them as patient needs change.
  • Administer medication as prescribed by the physician.
  • Reduce rehospitalizations by front-loading visits for high-risk patients and teaching patients preventive measures and sound decision-making.
  • Promote continuity of care through appropriate admissions, transfers, and discharges.
  • Counsel patients and family members about disease or injury processes and management.
  • Involve patients and families in developing plans of care.
  • Listen to patients and family members.
  • Oversee and supervise the total care provided by nurse aides and licensed practical nurses.
  • Evaluate treatments and patients' medical conditions according to the plan of care.
  • Determine whether the level of care meets patients' needs.
  • Communicate supervisory visit results to licensed practical nurses and nurse aides.
  • Complete clinical documentation in accordance with agency protocol and Medicare and federal guidelines.
  • Document treatment, assessments, patient education, subsequent visits, discharge visits, evaluation and recertification visits, resumption-of-care visits, and start-of-care visits within the required timeframes.
  • Maintain an active Registered Nurse license.
  • Communicate schedule changes outside the normal schedule to the scheduler.
  • Participate in on-call requirements and case conferences.
  • Attend in-service training and mandatory agency meetings.
  • Drive to patients' primary locations for scheduled visits and complete daily attendance at assigned visit locations.
  • Fulfill Registered Nurse visits within the assigned geographic location according to patient needs and federal and state specifications.
  • Perform other duties as assigned.
