Requirements
  • Membership in good standing with the applicable regulatory body, including the College of Nurses of Ontario, College of Physiotherapists of Ontario, College of Occupational Therapists of Ontario, College of Audiologists and Speech Language Pathologists of Ontario, Ontario College of Social Workers and Social Service Workers, or College of Dietitians of Ontario.
  • At least two years of recent experience in community health or a related field.
  • Knowledge of the health care delivery system and community resources.
  • Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills.
  • Effective time management, prioritization, and organizational skills, with the ability to work independently and cooperatively in a busy multidisciplinary environment.
  • Ability to accurately complete required documentation, reports, and forms.
  • A valid driver's licence and access to a reliable vehicle.
  • Proficiency in a Windows environment.
  • Proof of COVID-19 vaccination status must be submitted before the start date as a condition of employment.
Responsibilities
  • Assess patient care needs and determine eligibility for services in collaboration with patients and their families.
  • Develop individual care and service plans.
  • Link patients with service providers.
  • Coordinate and monitor care plan delivery.
  • Establish helping relationships with patients and their families.
  • Balance patient needs and choices with available resources while respecting patients' values and preferences.
Desired Qualifications
  • Experience working with diverse patient groups, including multicultural, homeless, palliative, acquired brain injury, mental health, geriatric, or pediatric populations.
  • Case management experience or recent related community experience.
  • Ability to speak French or another second language.
