Requirements
  • Board-Certified Nurse Practitioner or Physician’s Assistant
  • Active license to practice in the state of Ohio
  • AANC or AANP certification for Nurse Practitioners
  • Active DEA Controlled Substance Registration or eligible for application
  • Certified in Basic Life Support for Healthcare Providers
  • Maintenance of professional credentialing and CME standards
  • Experience providing clinical services to individuals with chronic medical and behavioral health conditions
  • Proficiency in clinical nursing skills such as wound assessment, blood drawing, and care plan reinforcement for chronic conditions
  • Adaptable, organized, efficient, independent self-starter and problem-solver
  • Proficient experience using and navigating an EMR
Responsibilities
  • Co-manage a panel of members for longitudinal primary care, care management, and care coordination
  • Provide comprehensive care management, chronic disease management, urgent home-based and community-based primary care visits
  • Perform episodic urgent medical/behavioral health visits for members on the panel
  • Conduct home visits for panel members within 48 hours of discharge from a care facility
  • Interface with specialists, hospitals, and community-based organizations
  • Monitor and address clinical quality gaps in care
  • Assist in the development and enhancement of clinical programs and initiatives
  • Attend team huddles and case conferences to advise on health needs
