The Housing Specialist coordinates all aspects of the housing eligibility and determination process including review of assessment and H2O evaluation documentation, requesting additional information from outside agencies, and contacting referring party for clarifying information as needed. The Housing Specialist prepares case files for review and ensures all determination timelines are met. Conducts care coordination for members with significant barriers to accessing housing.
