Requirements
- RN licensure required in California
- Bachelor's degree
- Two to five years of experience
- Relocation is required
- Demonstrates excellent verbal, written and computer skills
- Ability to work in team-based environments

- Ability to generate solutions to problematic situations
- Knowledge of internal policies and Federal/State regulatory requirements related to discharge planning and social services
- Basic knowledge of department budget management and other fiscal responsibilities
- Understands regulatory requirements in the area of responsibility
Responsibilities
- Performs psychosocial assessments and develops a plan to meet the needs of patients with complex social issue/problems, and arranges post-discharge services for patients as needed
- Abuse and neglect case handling including making referrals to state and federal agencies
- Participates in the weekly Complex Case Review meeting and the daily patient care conferences
- Provides resources and makes referrals to community agencies for ongoing counseling and/or continued care
- Maintains an updated list of providers for post-discharge care
- Collaborative communication with post-discharge providers
- Update Case Managers on variance to post-discharge plans
- Monitor and provide reports to the Director of Case Management and hospital leadership as requested
- Maintains knowledge of regulatory requirements and community resources
- And other duties as assigned
