Requirements
  • A registered nurse license in good standing with the State Board of Nursing in the state of employment is required.
  • American Health Association Healthcare Provider CPR certification is required, or must be obtained within thirty days of hire, with biannual recertification.
  • Formal training in management of the aggressive patient is required within sixty days of hire, with annual recertification.
  • The employee may be required to hold a qualified medical professional designation and must be deemed competent to serve as a qualified medical professional.
  • Employees at Child/Adolescent Day Treatment programs licensed under HFS §40 must have training in psychiatric nursing, including work with children with mental health disorders, or one year of experience working in a clinical setting with these children.
  • The registered nurse must complete initial patient assessments and document them as required.
  • The registered nurse must collect, record, and analyze pertinent admission-assessment data within the prescribed timeframe.
  • The registered nurse must involve patient support systems in assessment and documentation when appropriate.
  • The registered nurse must explain patient rights, obtain required signatures and consents, and protect patient privacy and confidentiality.
  • The registered nurse must participate in planning and modifying patient plans of care and represent the nursing component during care conferences.
  • The registered nurse must identify, interpret, and document information from nursing interviews, observations, physical assessments, and diagnostic data.
  • The registered nurse must review laboratory data, evaluate fall risk, identify risks of self-abuse, suicidality, or assaultive behavior, and document changes in patient status and interventions.
  • The registered nurse must administer medications safely and accurately, transcribe physician medication orders, monitor medication effects and adverse reactions, and provide medication education.
  • The registered nurse must identify patient-care problems, abrupt changes, or impending instability and intervene appropriately.
  • The registered nurse must use appropriate de-escalation techniques, re-evaluate safety levels, and identify alcohol withdrawal syndrome, extrapyramidal side effects, neuroleptic malignant syndrome, and significant cardiac or respiratory symptoms.
  • The registered nurse must implement age-appropriate patient and family education and document it in the patient record and treatment plan.
  • The registered nurse must follow established care plans, hospital policies, unit standards, safety standards, and infection-control standards.
  • The registered nurse must participate in continuing education, in-service programs, performance improvement activities, committees, and unit projects.
  • The registered nurse must identify unit-system, communication, educational-equipment, and supply needs and suggest solutions to the Clinical Services Manager.
  • The registered nurse must coordinate unit activities when assuming the charge nurse role, facilitate admissions, manage the therapeutic milieu, plan patient-care assignments, and communicate unit status and projected needs.
  • The registered nurse must supervise and delegate tasks to LPNs and psychiatric technicians based on their preparation and demonstrated abilities.
  • The registered nurse must maintain a primary caseload and provide direction for patient care.
  • The registered nurse must promote department goals, maintain policies and procedures, educate new staff as directed, and support student interns as directed.
  • The registered nurse must conduct themselves professionally, comply with hospital policies, communicate concerns and solutions, and accept professional accountability for their practice.
  • The position requires full-time availability for forty hours per week, Monday through Friday, without weekends, holidays, or on-call responsibilities.
Responsibilities
  • Provide professional nursing care to patients in the PHP/IOP outpatient clinic so they can achieve, regain, and maintain maximum physical, emotional, or social functioning.
  • Complete initial assessments and document required patient information.
  • Collect, record, and analyze admission-assessment data, including patient strengths and limitations, cultural, spiritual, and ethnic factors, discharge needs, medical and physical status, medication history, and age-specific needs.
  • Involve family and friends in assessment and documentation when appropriate.
  • Advocate for patients, explain patient rights, obtain consents, assist with orientation, and follow unit safety and admission-search procedures.
  • Initiate and update treatment plans and documentation, participate in care conferences, and develop treatment goals with patients and families.
  • Identify and document patient information from interviews, observations, physical assessments, and diagnostic data, and confer with other healthcare professionals as appropriate.
  • Implement patient care, including medication administration, medication monitoring, patient education, and adherence to care plans and hospital standards.
  • Identify patient-care problems and instability, exercise clinical leadership, and intervene to prevent adverse outcomes.
  • Identify and address patient and family health-education needs before discharge.
  • Organize care activities according to patient priorities, unit needs, and time constraints.
  • Participate in unit standards of care, performance improvement programs, hospital committees, and continuing education.
  • Assume charge nurse responsibilities, including coordinating unit activities, facilitating admissions, managing the therapeutic milieu, planning assignments, and supervising or delegating tasks.
  • Maintain a primary caseload and direct patient care according to unit standards.
  • Promote department goals and the hospital mission, maintain regulatory requirements, and educate staff regarding applicable regulations.
  • Conduct self in a professional manner and communicate respectfully with patients, families, physicians, visitors, vendors, and hospital employees.
Desired Qualifications
  • A Bachelor’s degree in nursing (BSN) is preferred.
  • Previous psychiatric experience with children, adolescents, and adults is preferred.
