Job Title: Memory Care Coordinator

Job Summary:

We are seeking a compassionate and dedicated Memory Care Coordinator to join our team. The ideal candidate will be responsible for overseeing the memory care program, ensuring the highest quality of care for residents with memory impairments. This role requires strong leadership skills, excellent communication abilities, and a deep understanding of memory care practices.

Key Responsibilities:

- Develop and implement individualized care plans for residents with memory impairments.

- Coordinate and oversee daily activities and programs that promote cognitive engagement and social interaction.

- Train and supervise staff in memory care best practices and techniques.

- Collaborate with families to provide support and education regarding memory care needs.

- Monitor and assess residents' progress and adjust care plans as necessary.

- Ensure compliance with all regulatory requirements and organizational policies.

Skills and Qualifications:

- Bachelor's degree in nursing, social work, gerontology, or a related field preferred.

- Previous experience in memory care or a similar role is highly desirable.

- Strong understanding of dementia and other memory-related conditions.

- Excellent communication and interpersonal skills.

- Ability to work collaboratively with a diverse team and build positive relationships with residents and families.

- Compassionate and patient demeanor with a commitment to providing high-quality care.

Requirements

 PHYSICAL QUALIFICATIONS: • Walk/Stand – must be able to continuously walk and stand. • Environment Condition – must be able to perform work both inside and outside. • Sit – sit infrequently. • Lift - constantly 0-10 pounds, frequently 11 – 50 pounds, occasionally 51 – 100 pounds. • Carry - constantly 0-10 pounds, frequently 11 – 50 pounds, occasionally 51 – 100  pounds. • Push - constantly 0-10 pounds, frequently 11 – 50 pounds, occasionally 51 – 100 pounds. • Climb – must be able to climb stairs on a frequent basis, with the opportunity to utilize  the elevator as an alternate resource if available. • Bend – must be able to bend at the waist, knees, hips, and spine on a frequent basis; may  require frequently manipulating weights of 25 pounds or more. • Squat – must be able to squat on a frequent basis to reach below the knee, may require  lifting of 50 pounds or less. • Twist or rotate – must be able to twist or rotate occasionally at the trunk, hips, knees, and  neck, manipulating weights of 25 pounds or more. • Reach– must be able to reach on a frequent basis. May occasionally reach overhead,  requiring manipulation of weight of 10 pounds or less, and below waist manipulating  weight of 25 pounds or more.
