  • A Qualified Professional (10A NCAC 27G .0104) must have one of the following: Bachelor's degree (non-Human Services) with 4 years Full time OR 8 years Part time applicable experience OR Bachelor's degree (Human Services) with 2 years Full time OR 4 years Part time applicable experience OR Master's degree (Human Services) with 1 year Full time OR 2 years Part time applicable experience.
Responsibilities
  • Provide initial and ongoing assessment to support identified individuals with accessing and applying for Disability benefits
  • Advocate, obtain and community with other providers and natural supports as needed to support the individual’s application for benefits
  • Assist individuals in understanding the disability benefits application process, including eligibility criteria, required documentation, and the timelines involved.
  • Provide one-on-one guidance and support in preparing application materials, ensuring completeness and accuracy.
  • Assist with gathering and submitting necessary medical records and other supporting documents required for disability benefit applications.
  • Ensure all documents are properly organized, complete, and submitted to the appropriate government agencies.
  • Follow up on the status of applications and assist patients with any additional paperwork or steps needed.
  • Assist patients in appealing denied claims and guide them through the steps of the appeals process.
  • Maintain accurate and confidential records, documenting all interactions, progress, and updates on applications.
  • Track and monitor each individual’s progress throughout the application and appeal processes.
  • Answer questions and clarify concerns regarding the disability benefits system
  • Use a range of communication skills and strategies to establish a mutual relationship with the individual, staff, co-workers, supervisors, other stakeholders and people who are important to the individuals receiving services
  • Identify opportunities for community connections and development/enhancement of natural support networks for people served. Work with service staff to educate people served on these options and access opportunities
  • Prepare and submit required or requested reports, documents, assessments, evaluations and paperwork. Review and sign off on clinical documentation as required. Release confidential information within assigned departments when authorized.
  • Continuously use person-centered approaches and positive approach strategies when interacting with individuals.
  • Provide exceptional customer service by ensuring that individuals feel supported and informed during the entire disability application process.
  • Serve as a point of contact for individuals seeking assistance with disability benefits and provide updates in a timely manner.
  • Attend and actively participate in meetings and training as required.
  • Maintain certification in all agency, state, and federal training requirements.
  • Demonstrate knowledge of emergency procedures including preventative and crisis services.
  • Work with supervisor on the continual assessment and enhancement of services provided throughout the agency.
  • Demonstrate knowledge of and comply with all agency policies and procedures, as well as state and federal statutes and regulations related to specific program areas.
  • Follow service definition guidelines for services being provided.
  • Complete all other relevant responsibilities as assigned by the supervisor.
  • Driving and travel may be required.

Fontana Reports $835.9M Gross Profit Growth

Fontana Limited reported a Q3 2025 gross profit of $835.9 million, a 15.2% increase from the previous year, driven by a 15.3% rise in revenue to $2.2 billion.
