Requirements
  • An active Michigan occupational therapist license in good standing is required.
  • Strong clinical judgment and the ability to work independently in the field are required.
  • The ability to escalate changes in patient condition to a registered nurse or physician and clearly document the rationale is required.
  • Strong communication and documentation skills are required.
  • Reliable follow-through on orders, visit frequencies, and patient goals is required.
  • A valid driver's license, auto insurance, reliable transportation, and comfort with daily travel across the assigned territory are required.
  • The ability to carry and transport basic therapy equipment as needed is required.
  • Competency in core home health occupational therapy scope, including activities of daily living and instrumental activities of daily living training, home safety, adaptive equipment, and caregiver education, is required.
Responsibilities
  • Complete in-home occupational therapy evaluations and deliver skilled occupational therapy visits.
  • Build and execute plans of care focused on activities of daily living, instrumental activities of daily living, safety and fall prevention, and functional independence.
  • Recommend and train patients and caregivers on adaptive equipment and home modifications as needed.
  • Coordinate with nursing and the broader care team when patient condition or needs change.
  • Document promptly and thoroughly in a defensible, efficient, and consistent manner.
  • Carry a full, high-quality caseload and work toward approximately 28 productivity points per week at the target visit quality.
  • Own patient outcomes by driving low avoidable-hospitalization rates and strong patient-experience scores and participating in case conferences and team huddles.
  • Build trust with referral sources through responsive, high-quality patient care.
  • Establish charting, visit cadence, and clinical judgment standards that can serve as templates for future clinicians.
Desired Qualifications
  • Home health experience is strongly preferred.
  • Deeper occupational therapy experience with neuro, cerebrovascular accident, fall-risk, dementia, or caregiver-training populations is preferred.
  • Strong organization and time-management skills are preferred.
