Requirements
  • ANCC or AANP certification for Nurse Practitioners
  • Active DEA Controlled Substance Registration or eligible for application
  • Certified in Basic Life Support for Healthcare Providers
  • Maintenance of professional credentialing and CME standards
  • Work a full-time 40 hour week, Monday-Friday
  • Experience providing clinical services to individuals with chronic medical and behavioral health conditions
  • Skills for independent clinical decision-making
  • Adaptable, organized, efficient, independent, problem-solver, leader, strategic thinker, mentor
  • Excitement for technology in healthcare
  • Availability for urgent member needs outside of normal business hours
  • Proficient in using and navigating an EMR
  • Unrestricted driver’s license and vehicle for daily use
Responsibilities
  • Co-manage a panel of members for holistic health improvement
  • Provide comprehensive care management and primary care visits
  • Perform urgent medical/behavioral health visits and home visits
  • Conduct post-discharge visits for panel members
  • Interface with specialists, hospitals, and community organizations
  • Monitor and address clinical quality gaps in care
  • Utilize care facilitation platform and EMR for data collection
  • Assist in the development of clinical programs and initiatives
  • Attend team huddles and case conferences
