Requirements
- Nurse Practitioner or Physician Assistant with, or eligible for, Vermont licensure.
- Current Basic Life Support certification is required.
- Nurse Practitioners must graduate from a Nurse Practitioner program with a Master's or Doctoral degree approved by the Vermont Board of Nursing or a national accrediting agency.
- Nurse Practitioners must maintain active registered nurse licensure and active, unrestricted Advanced Practice Registered Nurse licensure in Vermont.
- Physician Assistants must successfully complete a Physician Assistant program at an institution accredited by the Accreditation and Review Commission on Education for the Physician Assistant.
- Physician Assistants must hold a Bachelor's degree.
- Physician Assistants must hold current certification by the National Commission on Certification of Physician Assistants.
- Physician Assistants must hold active, unrestricted Vermont licensure to practice as a Physician Assistant.
- Nurse Practitioners and Physician Assistants must hold Drug Enforcement Administration registration or be eligible for registration.
Responsibilities
- Provide comprehensive inpatient care in a skilled nursing and rehabilitation setting.
- Manage a panel of approximately 6–8 patients per day.
- Assess and manage acute changes, chronic conditions, and post-hospital transitions.
- Lead daily rounding, including review and triage of overnight events with nursing staff.
- Support safe and efficient discharges back to primary care providers.
- Collaborate closely with physicians, nurses, rehabilitation therapists, and care managers.
- Provide rotating weekend call coverage from home from 7:00 AM to 7:00 PM.
Desired Qualifications
- At least 2 years of experience in primary care, hospital medicine, urgent care, or post-acute care.
- Comfort managing medically complex adult or geriatric populations.
- Ability to work independently while collaborating closely with a multidisciplinary team.
- Prior advanced practice provider clinical experience is strongly preferred.
- Registered Nurse First Assistant certification is strongly recommended.
- A Master's degree is preferred for Physician Assistants.
