Responsibilities
Assess patients for functional limitations and develop therapy plans of treatment with specific goals in collaboration with patients, families, nurses, physicians, and other health team members.
Provide direct care to treat medical or surgical conditions using accepted physical therapy modalities.
Instruct and supervise patients, families, and home health aides on the home exercise program and other appropriate aspects of patient care, and document their responses to instruction.
Continually re-evaluate patients’ conditions and modify plans of treatment as appropriate.
Identify and report significant clinical information to physicians, the clinical manager, the primary nurse coordinating services, and other disciplines.
Document all clinical evaluations, treatments, and patient responses to therapy.
Desired Qualifications
Home care experience is preferred.
Experience with OASIS assessments is preferred.

Physical Therapist Position Description:
Our physical therapists assess patients for functional limitations and develops a therapy plan of treatment with specific goals in collaboration with the patient, family, nurse, physician and other health team members. Ability to utilize industry technology such as a tablet when in the field with patients.
Experience with Oasis assessments is preferred.
Ability to drive with automobile in good working condition and available for use in the field.
