Requirements
  • Graduation from an accredited school of nursing or equivalent education for licensing is required.
  • Three years of acute care experience are required.
  • An applicable Registered Nurse license is required for the state of work location: Illinois, Missouri, Oklahoma, or Wisconsin, including a compact-state Registered Nurse license where applicable.
  • The role requires frequent lifting, carrying, pushing, and pulling of objects weighing 0–25 pounds; frequent sitting, standing, walking, reaching, and repetitive foot, leg, hand, and arm movements; frequent near and far vision and color identification; frequent hearing and oral communication; and frequent keyboard use or data entry.
  • The role requires occasional bending, stooping, kneeling, squatting, twisting, gripping, and lifting, carrying, pushing, or pulling objects weighing 25–50 pounds, as well as rare climbing.
Responsibilities
  • Ensure compliance with Condition of Participation discharge planning requirements.
  • Identify and escalate gaps and barriers related to inpatient and observation length of stay.
  • Assess patients at high risk for readmission, identify root causes for readmission, develop plans, and collaborate with the multidisciplinary team regarding discharge planning to the next level of care.
  • Participate with other team members in opportunities to improve standard work.
  • Support utilization management, emergency department case management, and admission processes when assigned.
  • Act as a case manager liaison with post-acute providers, including skilled nursing facilities, rehabilitation facilities, and home health providers, when assigned.
  • Apply evidence-based practice and scientific knowledge in health care to nursing practice, ensuring care is based on patients' age-specific and clinical needs.
  • Follow professional nursing standards and use the American Nurses Association Code of Ethics for Nurses to guide responses to patients' current and evolving health and nursing needs.
  • Work in a constant state of alertness and in a safe manner.
  • Perform other duties as assigned.
