Requirements

Responsibilities
  • Assist residents in obtaining personal documents such as identification and legal documentation as needed.
  • Expected to have contact with the residents on their caseload at least once a week in conjunction with the case manager of the case to assess and move the case forward toward a timely release.
  • Assists in gathering sponsor background checks, and fingerprinting checks.
  • Gathers, uploads to the ORR UC Portal, and files all sponsor documentation.
  • Attends and participates in cottage meetings and unit meetings.
  • Responsible for documenting case notes for all services rendered.
  • Accurate and timely completion of assigned ORR documentation related to community support and linkages as per identified ORR objectives.
  • Assist UC in obtaining additional services needed including but not limited to community linkages, education, specialized groups, and mental health/ health services.
  • Assists in conducting legal resource presentations, coordinates with the resident’s legal team, and provides support in completing the initial assessments with residents.
  • In place of the case manager, communicates all updates to the clinician and treatment team members.
  • In conjunction with the case manager, coordinates discharge planning tasks.
  • Provides support to external Case Managers (i.e., TPG services), when needed.
  • Utilizes strengths-based, trauma-informed, relationship-based, culturally responsive, and client and family-centered approaches.
  • Weekly updates the Case Management tracker to ensure all services are meeting the deadlines.
  • Maintain up-to-date on policies related to case management and family reunification support per ORR regulations.
  • Audits case files to ensure compliance with stakeholders and funders.
