Requirements
  • A current Licensed Practical Nurse license in the state of Ohio is required.
  • At least one year of experience as a Licensed Practical Nurse in an acute care setting is required.
  • The candidate must be able to communicate effectively with patients, families, physicians, and co-workers.
  • The candidate must remain calm, patient, accommodating, compassionate, and caring when working with patients.
  • The candidate must have knowledge of nursing best practices.
  • The candidate must be able to make appropriate nursing judgments.
  • The candidate must be able to identify situations and handle them with the best possible solution.
  • The candidate must be detail-oriented and observant.
  • The candidate must have a disciplined work style and be able to prioritize tasks and work in a timely manner.
  • The candidate must be able to follow directions and work as a team member.
  • The candidate must have a valid driver's license and auto insurance listing the candidate as a driver.
  • The candidate must meet all physical position requirements.
  • The candidate must provide evidence of a current negative tuberculin skin test or chest X-ray, or receive tuberculosis testing at orientation, with annual testing thereafter unless prior documentation is available.
  • The candidate must be free of physical or medical conditions that would limit or restrict performance of the listed job functions.
  • The candidate must be able to read, speak, and write English.
  • The candidate must be able to use communications systems, computers, and personal digital assistants.
  • The candidate must have hand and finger dexterity, hearing, and vision sufficient to perform the job.
  • The candidate must be able to travel for the position and drive to assigned patient locations.
  • The candidate must be able to lift up to 50 pounds with or without assistance.
Responsibilities
  • Provide comprehensive skilled nursing care in patients' homes as ordered by the attending physician.
  • Perform care by following order sets and goals established by the registered nurse.
  • Observe and monitor patient conditions and notify the registered nurse case manager of changes.
  • Administer medications as prescribed by the physician.
  • Counsel patients and family members about disease or injury processes and how to manage them.
  • Incorporate patients and family members into development of the plan of care.
  • Teach patients preventative measures and support decisions that help decrease rehospitalizations.
  • Listen to patients and family members.
  • Complete clinical documentation in accordance with agency protocol and Medicare and federal guidelines.
  • Document all aspects of treatment, assessments, and patient education.
  • Maintain an active Licensed Practical Nurse license in the state of employment.
  • Communicate schedule changes outside the normal schedule to the scheduler.
  • Participate in on-call requirements and case conferences.
  • Attend in-service training and mandatory agency meetings.
  • Maintain a professional appearance as a company representative.
  • Follow agency policies and procedures.
  • Complete daily visits at assigned patient locations.
  • Complete and submit documentation for subsequent visits within 24 hours and within specified company requirements.
  • Provide skilled care within the Licensed Practical Nurse scope of practice.
  • Fulfill Licensed Practical Nurse visits in the assigned geographic location according to patient needs and federal and state specifications.
Desired Qualifications
  • Two years of experience as a Licensed Practical Nurse is preferred.
  • Home care experience is desired.
