Requirements
  • A Bachelor's degree in Nursing is required.
  • Five years of current clinical experience is required, ideally in medical-surgical, home care, hospice, community health, rehabilitation, long-term care, cardiovascular, diabetes, renal or pulmonary diseases, oncology, or geriatrics.
  • Experience in a managed care, home health, or hospital environment is required.
  • An unrestricted state-licensed registered nurse license is required.
  • Knowledge of the scope of nursing practice in the states where licensed is required.
  • Current clinical knowledge of chronic diseases, heart disease, chronic obstructive pulmonary disease, diabetes, renal disease, and other comorbidities is required.
  • Ability to use sound judgment to identify subtle changes and anticipate and intervene to prevent problems and complications is required.
  • Ability to manage multiple priorities and changing schedules is required.
  • Strong written and verbal communication skills are required.
  • Strong organizational and time-management skills are required.
Responsibilities
  • Complete comprehensive advanced-level nursing assessments for new and existing members, including focused physical examinations with attention to complex care and comorbidities.
  • Apply evidence-based best-practice standards of care for chronically ill members.
  • Provide care management and care coordination according to standards and definitions established by the Care Management Society of America, the National Committee for Quality Assurance, TRICARE, and the Centers for Medicare & Medicaid Services.
  • Apply evidence-based criteria, including Centers for Medicare & Medicaid Services criteria and Healthcare Effectiveness Data and Information Set quality measures, to care-management processes and interventions.
  • Manage care within benefits structures for each line of business and perform functions in compliance with Department of Defense, Centers for Medicare & Medicaid Services, URAC, and National Committee for Quality Assurance regulations.
  • Complete and document initial member assessments, screenings, care-transition assessments, encounters, and follow-up in the case-management medical record.
  • Collaborate with members' primary care providers, specialists, medical directors, and community-service agencies to address member-centered concerns.
  • Engage, assess, and provide interventions and support to members, families, and caregivers addressing medical, health, social, psychological, environmental, and lifestyle issues.
  • Document assessments, encounters, and other care-management activities in the case-management medical record in a timely manner.
  • Ensure written communications, member records, and forms are complete, clear, organized, legible, professional, and compliant with care-management standards.
  • Develop and present case studies, participate in clinical rounds and in-services, and support staff education and training.
  • Demonstrate respectful, professional, and ethical conduct and provide customer service to all contacts.
  • Assist with process improvements and support quality initiatives to improve efficiency and outcomes.
  • Support projects and complete additional assignments as required.
Desired Qualifications
  • A Master of Science in Nursing is preferred.
  • Certified Case Manager certification is preferred.

Martin'S Point Health Care Risk Adjustment Settlement

PORTLAND, Maine, July 31, 2023 /PRNewswire/ -- Today, Martin's Point Health Care announced that it has agreed to a settlement with the U.S. Department of Justice (DOJ) to resolve an investigation related to Martin's Point's Risk Adjustment practices for the payment years 2016-2019. The claims resolved by the settlement are allegations only, and there has been no determination of liability. The settlement is not related to member care or the payment of member claims.Martin's Point worked collaboratively with the DOJ during the course of the investigation. Despite denying liability for the litigation claims at issue, Martin's Point ultimately determined that settlement of this matter was appropriate rather than engaging in the cost and uncertainty of protracted litigation.A spokesperson for the organization commented: "This settlement is not an admission of liability, it instead allows us to avoid the disruption, expense, and uncertainty of litigation. It is important to note that this investigation is unrelated to member care or payment of member claims
