Requirements
  • High school graduation is required; an associate or bachelor's degree is preferred.
  • Access to reliable transportation is required to travel throughout the service area and perform essential functions.
  • At least two years of experience within the last five years working in a social or recreational program providing and coordinating services for a frail or elderly population, or completion of Fallon Health's offered training program.
  • CPR and Alzheimer's certifications, or willingness to obtain them, is essential.
Responsibilities
  • Develop and implement a comprehensive activities program addressing participants’ physical, social, intellectual, artistic, and spiritual needs, using input from restorative service consultants.
  • Collaborate with the Site Director to implement programs that encourage teamwork and positive outcomes.
  • Provide educational and culturally competent activities, including exercise, arts and crafts, music appreciation, and spirituality.
  • Ensure participant choice and engagement when planning and conducting activities.
  • Build a supportive community and warm social atmosphere for participants at the adult day health center.
  • Incorporate Participant Advisory Committee feedback and Member Experience survey results into the activities program.
  • Assess individual abilities, needs, and preferences during the intake process.
  • Identify problems, establish realistic goals, and create treatment plans using therapeutic activities.
  • Participate in the interdisciplinary team process, daily interdisciplinary team meetings, care planning, and quality meetings as needed.
  • Reassess participant needs and communicate updates to interdisciplinary team members for care-plan revisions.
  • Document quarterly reviews of participant needs and activity participation in medical records.
  • Plan, organize, and implement daily individual and group activity schedules, or supervise others who implement them, tailored to frail elderly participants.
  • Schedule, post, and distribute the monthly activity calendar.
  • Supervise Life Enrichment Assistants, conduct documentation audits, provide performance-based feedback in one-on-one meetings, and support professional growth.
  • Develop and manage the volunteer program, including recruitment, orientation, supervision, handbook maintenance, and volunteer data and statistics.
  • Maintain knowledge of service-area community resources and collaborate with other Life Enrichment professionals.
  • Invite guests and potential partners to learn about PACE and contribute time and talent to the program.
  • Cultivate community resources and entertainers for use in the day center.
Desired Qualifications

  • Certification as an Activity Director.
