Requirements
  • At least one year of experience in healthcare, case management, homeless services, or working with vulnerable populations.
  • Employment eligibility verification is required.
  • Have or be able and willing to obtain CPR/First Aid training.
  • Successfully complete a tuberculosis test, background screening, and drug test as a condition of hire.
  • Have reliable transportation, a valid driver's license, proof of insurance and ownership for personal vehicles used during work duties, and the ability to qualify for PATH's insurance coverage.
  • Work effectively with colleagues, participants, and stakeholders while contributing to team goals.
  • Convey information clearly and effectively, verbally and in writing, to individuals and groups.
  • Maintain ethical and professional standards and professional boundaries.
  • Gather and interpret data, identify trends and patterns, and produce accurate, high-quality work.
  • Analyze and evaluate information, identify key issues, consider alternatives, and make logical decisions.
Responsibilities
  • Collaborate with the Quality Assurance Department to ensure programs comply with contracts and partnership agreements.
  • Under the supervision of the program manager, function as a contact representative for program matters.
  • Under the supervision of the program manager, provide administrative oversight of the team, which may include coverage for the program manager.
  • Support completion of critical incident reports as required by PATH policies or the contract.
  • Maintain a direct client caseload and support clients through various phases from street to housing.
  • Support program operations and the program manager.
  • Take action and make decisions proactively and take on additional responsibilities.
  • Generate innovative solutions to problems or challenges.
