Requirements
  • Bachelor’s degree in Dietetics, Nutrition, or a related field.
  • Registered Dietitian (RD) credential through the Commission on Dietetic Registration (CDR). California state licensure or eligibility.
  • Minimum of 1-2 years of experience as a Registered Dietitian, preferably in a skilled nursing or long-term care setting.
  • Strong assessment, planning, and evaluation skills.
  • Proficiency in using electronic health record (EHR) systems.
  • Ability to work independently and as part of a multidisciplinary team.
  • Strong communication, teaching, and problem-solving skills.

Responsibilities
  • Conduct comprehensive nutritional assessments for new admissions, quarterly reviews, and as needed for residents based on changes in health status.
  • Collaborate with the interdisciplinary team to develop and implement individualized nutrition care plans for residents, focusing on promoting overall health and wellness.
  • Ensure all residents receive diets that align with their medical conditions and dietary restrictions, including texture modifications, therapeutic diets, and supplementation as necessary.
  • Regularly monitor and evaluate residents’ nutritional status, intake, and weight, and make adjustments to care plans accordingly.
  • Maintain accurate and timely documentation of assessments, care plans, progress notes, and all other relevant information in compliance with state and federal regulations.
  • Provide nutrition counseling and education to residents, families, and staff as needed.
  • Ensure compliance with all applicable local, state, and federal regulations, as well as facility policies and procedures. Stay current with industry trends and evidence-based practice guidelines.
  • Assist in menu planning, development, and review to ensure nutritional adequacy, palatability, and resident satisfaction.
  • Participate in care conferences and communicate effectively with physicians, nursing staff, and other healthcare professionals to promote continuity of care.
