Requirements
  • Current Licensed Practical Nurse licensure in good standing in the state or states of practice.
  • At least one year of experience under Licensed Practical Nurse licensure.
  • Ability to collaborate daily with a team of clinicians.
  • Ability to work independently with phone, email, and written communication; meetings with healthcare professionals; and an understanding of state and federal rules and regulations.
  • Time management skills to manage patient visits, documentation, meetings, scheduled visits, required meetings, urgent needs, prioritization, and triage of multiple concerns.
  • Ability to learn an electronic medical records system.
  • Willingness to drive, reliable transportation, a valid driver's license, and auto insurance.
Responsibilities
  • Provide care and services for a group of patients in their residences, including private homes, skilled nursing facilities, and assisted living facilities.
  • Contribute as a member of the interdisciplinary team by providing expertise regarding psychosocial assessments, interventions, and community resources.
  • Work closely with patients on a one-to-one basis while receiving support from the patient care team.
