Requirements
  • At least 5 years of clinical practice as an Advanced Practice Provider, specifically a nurse practitioner or physician assistant.
  • At least 1 year of experience seeing post-discharge patients as an Advanced Practice Provider.
  • At least 2 years of previous management or team leadership experience.
  • An active and clear registered nurse and advanced practice registered nurse license or Physician Assistant license in the applicant's home state, along with national advanced practice registered nurse certification and/or the ability to obtain licenses in multiple states, is required at the time of hire and must be maintained during employment.
  • Ability and willingness to work Monday through Friday from 9:30 a.m. to 6:00 p.m. Eastern Time.
Responsibilities
  • Provide clinical care to patients during the 30-day post-discharge period, identify and fill gaps in care, escalate symptomatic patients through appropriate pathways, manage patients as indicated, and reduce preventable readmissions.
  • Provide clinical and operational oversight and accountability for the clinical performance of the Advanced Practice Provider organization in the Transitions of Care program.
  • Manage the performance of frontline Transitions of Care Advanced Practice Providers to ensure the team meets required targets.
  • Develop, track, and manage clinical and operational key performance indicators for the Transitions of Care Advanced Practice Providers.
  • Identify areas for improvement and implement strategies to achieve targeted metrics.
  • Manage, empower, inspire, and align the Transitions of Care Advanced Practice Provider team, including conducting biweekly one-on-one meetings with each direct report.
  • Set personalized goals for direct reports and review individual performance.
  • Collaborate with the program medical director and other Advanced Practice Provider managers to enhance team processes and workflows within the Advanced Practice Provider team and across Transitions of Care roles.
  • Assist with recruitment strategy, hiring, and onboarding of new Transitions of Care Advanced Practice Providers.
  • Develop clinical processes and policies for the program.
  • Monitor clinical resource allocation and execute changes to meet the practice's visit needs.
  • Oversee scheduling, including management of time-off requests, monitoring daily, weekly, and monthly schedule density, and managing schedule changes that affect patient scheduling.
  • Oversee onboarding guidance for new providers in clinical and process development in conjunction with the education and training team.
  • Serve as the liaison to Transitions of Care leadership regarding the status, issues, and opportunities within the Advanced Practice Provider organization.
Desired Qualifications
  • Experience performing visits over telehealth video platforms, managing post-transition patients, and identifying the need for immediate interventions.
  • Experience managing acute and chronic disease exacerbations, including congestive heart failure exacerbations, diabetic emergencies, chronic obstructive pulmonary disease exacerbations, and hypertensive emergencies.
  • Experience collaborating on process improvement projects with members outside the applicant's role.
  • Willingness to grow and change using Plan-Do-Study-Act principles.

Just months after announcing a new round of funding, the Medicare Advantage startup is in talks to raise another round that could value the company at $25 billion, according to sources familiar with the matter. That's a jump from the company's $16 billion valuation in a funding round raised earlier this year, according to sources. Devoted Health, which is based in Waltham, Massachusetts, was founded in 2017 by brothers and former healthcare executives Ed and Todd Park. The startup combines health insurance and medical care for elderly people enrolled in the Medicare Advantage market. The company's proprietary AI tool, Orinoco, helps coordinate medical care for members. Ed Park was previously the COO of athenahealth, and Todd Park was the chief technology officer of the United States in the Obama administration. "We do not comment on our capital raising activity," said a company spokesperson. The fundraising reflects growing investor enthusiasm for healthcare companies that use AI to improve both patient care and the economics of medicine. The back-to-back rounds could also signal investor confidence in Devoted's ability to scale in one of healthcare's largest and fastest-growing markets. Medicare Advantage has become a battleground for both incumbent insurers and venture-backed startups. Medicare Advantage is the private alternative to the traditional Medicare program that provides health coverage to people 65 and older and people with disabilities. The program has grown rapidly over the past decade, with enrollment more than doubling. That boom has attracted both insurance giants and venture-backed startups eager to tap into the hundreds of billions of dollars the federal government spends on the program annually. Devoted generates revenue primarily through government-funded Medicare Advantage premiums, making it part insurer and part healthcare provider. The company had 466,000 members as of January 2026, a year-over-year increase of 121%. Devoted Health's investors include Andreessen Horowitz, Venrock, General Catalyst, Iconiq, and Emerson Collective, according to PitchBook.
