Requirements
  • Master Degree in Social Work or Clinical Mental Health Counseling required. Must have graduated from an approved social work or counseling program.
  • Must be licensed by the State of Ohio Board of Counselors, Social Workers, and Marriage and Family Therapists (CSWMFT) LSW, LISW, LPC, or LPCC.
  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
  • Ability to write routine reports and correspondence.
  • To perform this job successfully, an individual should have the ability to gain knowledge of current practice management system, electronic medical record, Microsoft Word, Internet, and Intranet.
  • Reasoning ability: Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
Responsibilities
  • Provides integrated behavioral health services for referred patients in school-based health center and/or clinic settings, addressing mental health issues within the context of overall health and wellness.
  • Provide consultation, brief assessments, and solution-focused brief intervention to patients to identify strengths, set goals, and create actionable plans to overcome challenges
  • Develop and implement integrated wellness plans that align behavioral health strategies with primary care needs, ensuring a holistic approach to patient care.
  • Facilitate therapeutic interventions that are solution-focused, time-limited, and centered around patient strengths and goals.
  • Conduct crisis interventions when needed, ensuring patients receive immediate, focused support in times of acute distress.
  • Collaborate with primary care providers, specialty providers, care coordinators, and community agencies to ensure seamless care coordination, referrals, and follow-up.
  • Support patients with self-management techniques, empowering them to engage in goal-setting and problem-solving to address mental health and behavioral health challenges.
  • Provide education and training for patients and staff on mental health, wellness strategies, and integrated care models.
  • Maintains complete, accurate, and timely medical records, documenting brief assessments, intervention plans, patient progress, and telephone message summaries.
  • Ensure progress notes and any other special patient forms are completed following each patient encounter, adhering to organizational standards for documentation and compliance with HIPAA regulations.
  • Communicate with patients, referring physicians, and other care providers to coordinate treatment and ensure continuity of care.
  • Participate in health improvement activities, including collaborative health teams and patient care reviews.
  • Ensure all clinical and administrative duties meet accreditation and regulatory standards.
  • Engage in Quality Improvement (QI) initiatives and other program evaluations, contributing to ongoing enhancements in patient care and service delivery.
  • Participate in team meetings, case reviews, and committee activities to support integrated care efforts and improve overall service quality.
  • Help develop and refine programs aimed at addressing behavioral health needs, with an emphasis on brief, solution-focused interventions.
  • Contribute to risk management and patient safety activities, maintaining a commitment to patient well-being and care excellence.
Desired Qualifications
  • Two years experience (preferred) working with indigent populations and community based programs.
  • Bilingual (Spanish) preferred
