Requirements
  • Graduate of an accredited nursing program
  • Active and unrestricted Registered Nurse license in the State of Colorado
  • Strong clinical assessment, communication, and documentation skills
  • A compassionate, patient-centered approach to care
  • The ability to communicate effectively with patients, families, caregivers, physicians, and other members of the care team
  • Comfort working independently while remaining closely connected to a collaborative clinical team
  • Ability to travel by car within an assigned geographic area
  • Valid driver's license and active auto insurance
  • Comfort using common clinical, case management, and productivity software
  • New graduates are encouraged to apply
Responsibilities
  • Conduct in-home clinical assessments and help patients and caregivers successfully begin services
  • Develop, implement, and update individualized plans of care based on each patient's needs, goals, and clinical condition
  • Build ongoing relationships with patients and caregivers through scheduled home visits and regular clinical follow-up
  • Use your clinical judgment to identify changes in condition, safety concerns, and barriers to successful care
  • Coordinate care with physicians, caregivers, internal clinical teams, and community resources
  • Educate and coach patients and caregivers on care plans, best practices, health and safety needs, and available resources
  • Review caregiver documentation and respond appropriately to clinical changes or concerns
  • Advocate for patients and help ensure they receive safe, effective, person-centered care
  • Maintain accurate clinical documentation and ensure care is delivered in accordance with applicable regulatory requirements and company standards
  • Collaborate with a supportive interdisciplinary team to improve patient outcomes and the overall care experience
