Requirements
  • NYS LPN licensure.
  • Basic Life Support certification.
  • A high school diploma or GED.
  • Graduation from a formal technical or vocational Licensed Practical Nurse program.
  • At least one year of experience as an LPN working with clients with Substance Use Disorder and/or Serious Mental Illness.
  • Availability to work a flexible schedule as needed in response to participant and staff needs.
  • Computer literacy, including proficiency in Microsoft Office Suite and electronic health records.
  • Ability to put patients at ease and explain physician’s instructions.
  • Ability to multitask with strong organizational skills.
  • Experience working directly with people from diverse racial, ethnic, and socioeconomic backgrounds.
  • Knowledge of medical terminology.
  • Ability to work independently and as part of a team.
Responsibilities
  • Assist with patient care by checking vital signs, including blood pressure, pulse rate, breathing, temperature, and weight; discussing chief health concerns; administering injections; and performing tests.
  • Take patients’ medical histories, complete pretreatment formalities, and enter data into the computer system.
  • Follow physician and registered nurse orders when providing care.
  • Record detailed reports of care provided in patients’ records.
  • Complete charting, documentation, and maintenance of patients’ medical records.
  • Check, clean, and dress wounds.
  • Reinforce instructions provided to patients by the registered nurse or physician.
  • Notify the registered nurse or physician of significant findings or changes in a patient’s physical or mental condition.
  • Call pharmacies to order prescriptions.
  • Maintain confidentiality of patient records.
  • Pull and prepare patient charts.
  • Maintain a neat work area.
  • Stock patient rooms with supplies.
  • Provide emotional support as needed.
  • Initiate emergency assistance according to policies and procedures.
  • Attend staff development training and nursing conferences.
  • Keep nursing skills current and increase knowledge and productivity.
  • Perform other duties as requested.
