Requirements
  • Possess, at minimum, a Nursing Degree from an accredited college or university, or graduate from an approved Licensed Practical Nurse/Licensed Vocational Nurse program.

  • Assist in evacuating residents during emergency situations when necessary.
Responsibilities
  • Provide direct nursing care to residents and supervise the day-to-day nursing activities performed by nursing assistants.
  • Direct the day-to-day functions of nursing assistants in accordance with applicable rules, regulations, and guidelines.
  • Ensure nursing personnel comply with facility policies, procedures, job descriptions, and the Nursing Service Procedures Manual.
  • Review department policies, procedure manuals, and job descriptions and recommend revisions.
  • Plan shift services, programs, and activities with assigned nursing staff and support personnel.
  • Prepare written and oral reports and recommendations concerning shift activities.
  • Coordinate nursing services with other resident services to maintain each resident’s total care regimen.
  • Participate in the nursing service quality assurance program and facility inspections.
  • Review and update resident discharge plans and assist in planning nursing services for discharge.
  • Interpret department policies and procedures for personnel, residents, visitors, and government agencies.
  • Admit, transfer, and discharge residents; complete related forms, charts, and reports.
  • Maintain census reports, charge slips, medical forms, evaluations, studies, and other administrative records.
  • Protect user credentials and resident protected health information and report unauthorized access attempts.
  • Complete admission, transfer, discharge, charting, accident, incident, and other required documentation.
  • Receive telephone orders from physicians, transcribe orders, and maintain resident medical records.
  • Chart nursing notes, accident and incident reports, diet orders, and routine documentation accurately and promptly.
  • Prepare and administer physician-ordered medications and treatments after verifying resident identity.
  • Maintain medication, narcotic, supply, and equipment inventories and report discrepancies or shortages.
  • Review medication cards and notify physicians of automatic stop orders before the final dosage.
  • Dispose of drugs and narcotics in accordance with established procedures.
  • Participate in employee performance evaluations, determine shift staffing requirements, and recommend personnel actions to the Nurse Supervisor.
  • Inform the Nurse Supervisor of staffing needs, absences, complaints, grievances, and problem areas.
  • Develop work assignments and provide leadership to nursing personnel assigned to the unit or shift.
  • Make daily rounds to ensure assigned personnel perform work according to nursing standards.
  • Ensure nursing assistants are enrolled in or have graduated from approved training programs and that trainees receive direct licensed-nurse supervision.
  • Meet regularly with nursing personnel to identify and correct problems and improve services.
  • Receive and give nursing reports at the beginning and end of shift duty.
  • Inform personnel about new admissions and ensure rooms are ready for incoming residents.
  • Greet and orient newly admitted residents and their family members to the facility.
  • Make physician rounds and arrange diagnostic and therapeutic services as ordered.
  • Consult with physicians regarding resident care, treatment, and rehabilitation.
  • Review resident charts and care plans and ensure prescribed treatments are administered appropriately.
  • Coordinate social and activity programs with nursing schedules.
  • Notify physicians and next of kin about accidents, incidents, or changes in resident condition.
  • Carry out restorative and rehabilitative programs, including self-help and care.
  • Provide clinical services including catheterization, tube feedings, suctioning, dressing changes, colostomy and drainage-bag care, blood collection, massage, range-of-motion exercises, and care of the dead or dying.
  • Obtain specimens for laboratory testing and record temperature, pulse, respirations, and blood pressure.
  • Monitor seriously ill residents and ensure residents unable to call for help are checked frequently.
  • Check food brought by visitors against residents’ dietary allowances.
  • Meet with residents and family members and report concerns to the Nurse Supervisor.
  • Arrange transportation for discharged residents and escort residents to the pickup area.
  • Notify family members of resident deaths, contact funeral homes when requested, and follow post-mortem procedures.
  • Develop, plan, conduct, and schedule in-service training for nursing personnel.
  • Orient new employees to the shift, policies, procedures, job duties, and position requirements.
  • Standardize work methods and train personnel on blood and body-fluid exposure risks.
  • Assist with clinical supervision planning for nurse aide trainees.
  • Attend outside training, annual facility in-service programs, advance-directive training, and continuing education.
  • Monitor personnel compliance with safety regulations, dress codes, infection-control procedures, hand hygiene, and use of personal protective equipment.
  • Maintain clean and sanitary nursing stations, medication rooms, resident-care rooms, and treatment areas.
  • Participate in infection-control programs and procedures for communicable diseases and isolation precautions.
  • Conduct fire-safety and disaster-preparedness drills and support emergency resident evacuation.
  • Ensure safe operation of nursing equipment and that only trained and authorized personnel use it.
  • Monitor supply usage to prevent waste and maintain adequate medication, medical-supply, equipment, and personal-protective-equipment stock.
  • Maintain required safety data sheets for hazardous chemicals and report missing or illegible labels.
  • Review resident care plans daily and notify the Nurse Supervisor of required changes.
  • Ensure nurses’ notes reflect care-plan implementation and ensure CNAs review and follow resident care plans.
  • Assist the Resident Assessment/Care Plan Coordinator with scheduling and revising the Minimum Data Set, Resident Assessment Protocols, and triggers.
  • Provide data to the Quality Assurance and Assessment Committee as requested.
