Requirements
  • RN licensure in the state of Massachusetts is required.
  • Three to five years of experience in an acute care setting is required.
  • The role requires effective verbal and written communication, relationship-building techniques, and negotiation skills.
  • The role requires current knowledge of organizational policies, care transitions, clinical trends, regulatory requirements for clinical care, discharge planning, and authorization for post-acute services.
  • The role requires the ability to work in a light physical-demand environment, including walking or standing to a significant degree.
  • Staff must be vaccinated against influenza as a condition of employment.
Responsibilities
  • The RN Case Manager partners with the interdisciplinary care team to facilitate progression of care for hospitalized patients.
  • The RN Case Manager collaborates with the healthcare team to develop the plan of care and patient flow.
  • The RN Case Manager reviews cases within 24–48 hours or the next business day of admission or bed placement and each day throughout the stay to establish anticipated length of stay and transition planning needs.
  • The RN Case Manager collaborates with the medical team to formulate treatment plans that include care transitions and promote patient flow.
  • The RN Case Manager completes initial assessments of admissions and observation patients to identify barriers affecting length of stay and discharge planning, current resources, and future resources needed for successful transitions.
  • The RN Case Manager navigates the care delivery system and collaborates with physicians and clinical departments to ensure tests, treatments, consults, and procedures are appropriately indicated and performed timely.
  • The RN Case Manager communicates the plan of care to care team members and the patient or caregiver and intervenes when deviations occur.
  • The RN Case Manager communicates the plan of care, expected discharge date, and transition needs to patients, caregivers, and the care team.
  • The RN Case Manager creates and coordinates the overall transition plan of care with social workers, direct care providers, hospital departments, external service organizations, agencies, healthcare facilities, community care and navigation services, and patients and families or caregivers.
  • The RN Case Manager participates in daily multidisciplinary rounds and incorporates evidence-based practice milestones into the plan of care.
  • The RN Case Manager apprises the interdisciplinary team of estimated length of stay, care progression barriers, and anticipated disposition.
  • The RN Case Manager identifies team actions needed to facilitate the plan and ensures appropriate clinical follow-up and referrals to post-acute providers.
  • The RN Case Manager communicates the plan effectively to patients and families or caregivers and ensures they have resources for success after discharge.

  • The RN Case Manager documents avoidable days, case management assessments, and care plans thoroughly and timely according to department policy.
  • The RN Case Manager ensures provider documentation supports the anticipated discharge plan of care and escalates deviations to the Physician Advisor as appropriate.
  • The RN Case Manager documents the care plan clearly and concisely and communicates it to the interdisciplinary team and the patient or caregiver.
  • The RN Case Manager identifies and communicates issues affecting patient flow, patient satisfaction, safety, length-of-stay management, or outcomes to the department director or appropriate stakeholder.
  • The RN Case Manager serves as a resource for governmental and healthcare-industry regulations and ensures compliance standards are communicated to the interdisciplinary team.
  • The RN Case Manager informs patients and families or caregivers about the plan of care and its progression, facilitates communication with providers, and encourages open dialogue.
  • The RN Case Manager attends and contributes to departmental staff meetings and multidisciplinary committees, workgroups, and other committees as directed.
  • The RN Case Manager manages quality indicators including avoidable delays, length of stay, resource utilization, patient satisfaction, patient flow, outlier management, and readmissions, and suggests strategies to improve organizational and departmental performance.
Desired Qualifications
  • A Bachelor’s degree in nursing or another healthcare-related field is preferred.
  • ACM, CCM, or CMAC certification is preferred.

Beth Israel Lahey Health rolls out Heidi's AI scribing to all physicians after 90% report better patient presence

Beth Israel Lahey Health is rolling out Heidi's ambient AI scribing tool to all its physicians following a successful trial across its facilities. The health system, which serves over 6,000 providers across 14 hospitals and 175 primary care practices, prioritises addressing physician burnout, with nearly half of US physicians reporting burnout. During a six-month trial offering Heidi to 1,000 providers, the majority adopted the technology. Survey results showed 89% were satisfied with note quality, 90% felt more present with patients, 74% reduced after-hours work time, and 82% experienced reduced cognitive load during visits. The implementation demonstrates how ambient AI can deliver immediate relief to physicians whilst benefiting patients and the healthcare system. Heidi supports over 2 million consultations weekly globally and has raised $96.6 million from investors including Point72 Private Investments and Blackbird.
