Job Description
Provides continuum of care for patients from admission to discharge. Assesses social, emotional, and financial needs of the patient. Assists in developing discharge plans with patients and their families. This includes referrals to community resources and/or placement in an appropriate facility, following discharge.

Additional Job Description
Education: Master’s degree in a social work or social welfare program accredited by the Council on Social Work Education. Credentials: MSW licensure by the South Carolina Board of Social Work Examiners.Certification in case management preferred.
