Responsibilities
- Provides daily clinical assessment of transplant patients throughout all phases of care, demonstrating critical thinking skills and functioning with a high level of autonomy.
- Coordinates the evaluation process for pre-transplant referral patients to ensure effective use of hospital and clinical resources.
- Assesses post-transplant organ function and general health status and collaboratively determines the need for additional assessment or intervention.
- Facilitates the scheduling of diagnostic studies or procedures while using fiscal responsibility for patient and hospital costs.
- Participates in the development and implementation of protocols and standards of care for transplant patients with the multidisciplinary team.
- Assists nurses and other staff to assess, plan, implement, and evaluate the care of transplant patients.
- Monitors the processes and outcomes of routine care to determine quality outcomes and performance-improvement processes.
- Provides on-call coverage, including coordination of activities prior to transplant and medical triage for outpatients in collaboration with the physician or provider on call.
- Coordinates transplantation processes when a donor is available, including communication among physicians, organ procurement organizations, patients and families, emergency department, operating room, intensive care unit, blood bank, and histocompatibility laboratory teams.
- Reviews patients' medical care, including laboratory and procedural results, medication reconciliation, patient-issue assessment, and triage care with transplant providers.
- Completes documentation according to Centers for Medicare and Medicaid Services and United Network for Organ Sharing regulatory requirements.
- Communicates in a timely manner with the multidisciplinary team and patients and families regarding care issues.
- Collaborates with multidisciplinary team members and care teams involved in transplant-patient care.
- Maintains current transplant-patient records to facilitate communication.
- Plans nursing care and medical procedures with fiscal responsibility in collaboration with transplant administrators, financial coordinators, utilization review, and third-party case managers.
- Educates potential recipient and donor candidates, their families, and caregivers throughout the continuum of care.
- Develops patient-education materials or identifies external resources appropriate for all phases of transplantation care.
- Educates nurses and multidisciplinary care-team members about patient-care issues to promote positive outcomes for transplant patients.
- Provides education and consultation about transplantation and organ donation to the community.
- Serves as a resource person and preceptor in organ transplantation for graduate and undergraduate nursing programs.
- Participates in or leads team conferences, rounds, and meetings related to transplant-patient care or program issues.
- Interprets research and published data for nursing staff and colleagues concerning current clinical, legal, ethical, and economic issues in transplantation.
- Incorporates transplant-patient research into evidence-based practice and collaboratively determines its appropriateness with the medical team.
- Identifies areas for study and participates in or designs and conducts research.
- Serves as a consultant to nurses and other healthcare professionals regarding organ-transplant-patient care.
- Advises referring physicians and nurses regarding patient care in collaboration with surgical and medical teams.
- Serves as a community consultation resource regarding transplantation and organ donation.
- Consults with unit leadership to resolve issues relating to transplant patients.
- Models professional nursing through conduct, communication, mutual respect, leadership, ethical decision-making, critical thinking, and problem-solving.
- Supports and participates in strategic planning for university transplant, the medical center, nursing, and ancillary support services.
- Participates in university transplant departmental committees and hospital-wide committees, task forces, and meetings as indicated.
- Participates in organ-transplant program planning.
- Maintains responsibility for professional practice through participation in professional organizations, continuing education, and departmental meetings and education opportunities.
- Advances nursing and solid-organ-transplantation knowledge through exchange of ideas and knowledge in posters, newsletters, presentations, and publications.
Desired Qualifications
- Nursing experience in an intensive care environment, related nursing experience, or transplant experience is preferred.
- A Bachelor of Science in Nursing is preferred.
- Certified Clinical Transplant Coordinator certification is preferred.
- Advanced Cardiovascular Life Support certification is preferred.
