Requirements
  • A Bachelor's degree in Nutrition or a directly related field is required.
  • Registration as a Registered Dietitian through the Commission on Dietetic Registration is required.
  • Current state licensure is required where applicable.
  • Less than one year of clinical experience is required.
  • The position requires travel between assigned facilities and various community locations, as well as possible travel to training clinics and regional, Business Unit, and Corporate meetings.
  • The employee must maintain dietetic registration and continuing education hours as specified by the American Dietetic Association and applicable state licensure regulations.
Responsibilities
  • Work directly with a qualified renal dietitian under direct supervision to provide nutritional services to in-center patients until the required clinical experience is completed.
  • Participate in process-improvement activities to help patients achieve individualized goals determined by their physicians.
  • Calculate diet prescriptions according to standards of practice.
  • Evaluate patients' knowledge of diet and kidney disease.
  • Complete Nutrition History forms for use by the CMS-qualified dietitian in patient assessments.
  • Provide patient-specific nutrition education to patients, families, and caregivers regarding nutritional status, adequacy measures, fluid status, and nutrition-related laboratory parameters.
  • Review nutrition-related laboratory results and counsel patients or caregivers on appropriate actions.
  • Communicate laboratory results and recommended treatment changes to the interdisciplinary team, physicians, facility staff, and other patient-care providers as appropriate.
  • Monitor adherence and response to nutrition therapy and address issues affecting patients' ability to achieve their goals.
  • Review each patient's albumin level and body weight at least monthly, identify malnourished or at-risk patients, and collaborate on interventions and resources.
  • Identify candidates for physician referral for nutritional supplements when appropriate and permitted by disease-management agreements.
  • Assist with fluid and volume management and provide fluid and sodium education as appropriate.
  • Collaborate with the interdisciplinary team on mineral metabolism, renal bone disease, anemia and iron status, nutritional status, psychosocial status, vascular access, modality, rehabilitation status, and sodium/volume control.
  • Participate in Quality Assessment and Performance Improvement and Plan of Care meetings with a CMS-qualified dietitian.
  • Attend pertinent staff meetings.
  • Collaborate with the interdisciplinary team to keep work areas safe and clean.
  • Collaborate on pharmacy-related responsibilities, including setup, enrollment, and management.
  • Participate in lobby days.
  • Educate patients about emergency preparedness as it relates to food and nutrition.
  • Conduct staff in-services as needed or requested.
  • Adhere to FMCNA algorithms and medical-record policies.
  • Maintain and improve knowledge and skills for competent and innovative practice.
  • Escalate issues to the supervisor for resolution when necessary.
  • Assist with projects assigned by the direct supervisor and perform other related duties as assigned.
