Requirements
  • A current Physical Therapist Assistant license in the state of employment is required.
  • A minimum of one year of experience as a Physical Therapist Assistant in an acute setting is required.
  • Knowledge of physical therapy best practices is required.
  • The ability to make appropriate physical therapy judgments is required.
  • A valid driver's license and auto insurance listing the employee as a driver are required.
  • The employee must meet all physical position requirements and be free of physical or medical conditions that would limit or restrict essential functions.
  • Evidence of a current negative tuberculin skin test or X-ray, or tuberculosis testing at orientation, is required; annual testing is subsequently required unless prior documentation is provided.
  • The employee must be able to read, speak, and write English.
  • The employee must be able to use a computer or personal digital assistant, communicate by phone and in person, and perform the stated physical demands, including lifting up to 50 pounds and driving.
Responsibilities
  • Provide skilled services in patients' homes according to the Physical Therapist's plan of care.
  • Observe, record, and report patient reactions, changes, and progress.
  • Perform prescribed physical therapy procedures and diagnostic tests.
  • Treat patients to relieve pain, develop or restore function, and maintain maximum performance using physical means such as exercise, massage, heat, and water.
  • Assess functional problems and provide interventions in conjunction with the Physical Therapist to improve patient functional status.
  • Communicate with physicians, therapists, coworkers, schedulers, patients, and family members as needed.
  • Participate in case conferences to promote coordination of care.
  • Recommend prosthetic or orthotic devices, evaluate existing devices, make minor adjustments, or recommend modifications.
  • Educate staff on specific physical therapy modalities when appropriate.
  • Educate patients and families on therapy techniques, safety measures, and supportive activities.
  • Instruct patients and family members on the proper use of wheelchairs, braces, walkers, crutches, canes, and other prosthetic or orthotic devices.
  • Foster patient and family independence to prevent dependence on the agency.
  • Complete clinical documentation according to agency protocol and Medicare and federal guidelines.
  • Document treatment, assessments, and patient education.
  • Complete chart audits at the agency location as requested.
  • Participate in on-call requirements, case conferences, in-service trainings, mandatory agency meetings, and orientation or training of new employees.
  • Drive to patients' primary locations for scheduled visits and attend assigned visit locations daily.
  • Document subsequent, discharge, evaluation, recertification, and resumption-of-care visits within 24 hours, and start-of-care visits within 48 hours.
  • Complete and submit required documentation within company time requirements.
  • Follow the plan of care within the scope of practice for a licensed Physical Therapist Assistant.
