Requirements
  • A nursing degree from an accredited college or university, or graduation from an approved Licensed Practical Nurse/Licensed Vocational Nurse program.
  • A current, unencumbered, active license to practice as a Licensed Practical Nurse/Licensed Vocational Nurse in the state.
  • Current cardiopulmonary resuscitation certification.
  • Required continuing education and licensing must be maintained.
  • Good standing with the State Board of Nursing must be maintained.

Responsibilities
  • Provide direct nursing care to residents and supervise the day-to-day nursing activities performed by nursing assistants.
  • Direct the day-to-day functions of nursing assistants in accordance with applicable rules, regulations, and guidelines.
  • Ensure nursing personnel comply with facility policies, procedures, job descriptions, and the Nursing Service Procedures Manual.
  • Plan shift services, programs, and activities with assigned nursing and support personnel.
  • Prepare written and oral reports and recommendations concerning shift activities.
  • Coordinate nursing services with other resident services to maintain each resident’s total care regimen.
  • Participate in the nursing service quality assurance program and facility inspections.
  • Review and update resident discharge plans and assist with planning nursing services for discharge.
  • Admit, transfer, and discharge residents as required.
  • Complete medical forms, reports, evaluations, studies, charting, census reports, charge slips, and other administrative documentation.
  • Protect resident health information and facility information-system credentials, and report suspected unauthorized access or disclosure.
  • Complete and file admission, transfer, discharge, accident, incident, and other required records.
  • Receive telephone orders from physicians and transcribe physician orders to resident charts, medication cards, treatment plans, and care plans.
  • Chart nurses’ notes and all required resident care, accidents, incidents, orders, and treatments accurately and promptly.
  • Prepare and administer medications and treatments as ordered by physicians.
  • Verify resident identity before medication or treatment administration and prevent medication errors.
  • Maintain adequate medication, supply, and equipment stock and report shortages.
  • Maintain accurate narcotic records, report discrepancies, review medication cards, process automatic stop orders, and dispose of drugs and narcotics according to procedure.
  • Participate in employee performance evaluations, determine shift staffing requirements, and make staffing recommendations.
  • Inform the Nurse Supervisor of staffing needs and absentee call-ins.
  • Develop work assignments and provide leadership to nursing personnel assigned to the unit or shift.
  • Conduct daily rounds to ensure nursing personnel perform assignments according to nursing standards.
  • Ensure nursing assistants are enrolled in or have graduated from approved training programs and that nurse aide trainees are directly supervised by a licensed nurse.
  • Meet regularly with shift personnel to identify and correct problems and improve services.
  • Receive and give nursing reports at the beginning and end of shifts.
  • Report occupational exposures, suspected fraud, and other incidents according to facility procedures.
  • Inform personnel of new admissions and ensure rooms are ready for new residents.
  • Greet and orient newly admitted residents and family members.
  • Make physician rounds and arrange diagnostic and therapeutic services as ordered.
  • Consult with physicians regarding resident care, treatment, and rehabilitation.
  • Review resident charts for treatments, medications, diets, and other care requirements.
  • Implement nursing objectives and standards and verify that treatments are properly administered.
  • Evaluate residents’ physical and emotional status and notify physicians and next of kin of accidents or changes in condition.
  • Carry out restorative and rehabilitative programs, including self-help and care.
  • Administer catheterization, tube feedings, suction, dressing and bandage changes, colostomy and drainage-bag care, blood collection, massages, range-of-motion exercises, and end-of-life care as required.
  • Use restraints according to established policies and procedures.
  • Obtain sputum, urine, and other specimens for laboratory testing.
  • Take and record temperature, pulse, respiration, blood pressure, and other measurements.
  • Monitor seriously ill residents and ensure residents unable to call for help are checked frequently.
  • Check food brought by residents’ families or visitors against dietary allowances.
  • Ensure direct care follows resident care plans and wishes.
  • Meet with residents and family members and report problem areas to the Nurse Supervisor.
  • Arrange transportation for discharged residents and escort residents to the pickup area.
  • Inform family members of a resident’s death, contact funeral homes when requested, and follow post-mortem procedures.
  • Develop, plan, conduct, and schedule in-service training for nursing personnel.
  • Orient new employees to the shift, policies, procedures, job duties, and position requirements.
  • Assist with standardizing work methods and training personnel on blood and body-fluid exposure risks.
  • Assist the Director with clinical supervision planning for nurse aide trainees.
  • Attend outside training, annual facility in-service training, advance-directive training, and continuing education programs.
  • Monitor personnel compliance with safety regulations, dress codes, hand hygiene, infection control, and equipment procedures.
  • Maintain clean, safe, and sanitary nursing work areas, resident care rooms, and treatment areas.
  • Ensure appropriate personal protective equipment is available and used for exposure-prone procedures.
  • Participate in infection control programs and communicable-disease monitoring.
  • Ensure safe operation of nursing equipment and that only trained, authorized personnel operate it.
  • Monitor efficient use of nursing supplies and maintain required material safety data sheets for hazardous chemicals.
  • Review resident care plans daily and notify the Nurse Supervisor of needed revisions.
  • Ensure nurses’ notes document care-plan compliance and ensure certified nursing assistants follow resident care plans.
  • Assist with Minimum Data Set assessment and care-plan scheduling, revision, and implementation.
  • Report suspected or known fraud involving false billings, cost reports, or kickbacks.
  • Provide data to the Quality Assurance and Assessment Committee as requested.

PACS Group has appointed Carey P. Hendrickson as chief financial officer, effective 27 April 2026. Hendrickson brings nearly four decades of financial leadership across healthcare, senior living and media sectors. Most recently, Hendrickson served as CFO of U.S. Physical Therapy, overseeing 779 outpatient clinics across 44 states. He previously held CFO roles at Capital Senior Living Corporation, managing 128 communities across 23 states, and Belo Corp. He succeeds Mark Hancock, PACS's co-founder and executive vice chairman, who will retire by 30 June 2026 whilst remaining on the board as vice chairman. PACS Group has announced that co-founder and chief financial officer Mark Hancock will retire on 30 June 2026. He will continue serving on the company's board of directors as vice chairman. Hancock co-founded PACS in 2013 with Jason Murray, starting with two post-acute care facilities in San Diego. Under his leadership, the company expanded to 323 facilities across 17 states, serving over 31,700 patients daily and generating $5.29 billion in revenue in 2025, representing 29.3% year-on-year growth. Dr Conway currently serves as CEO of Optum, UnitedHealth Group's health services division with revenues exceeding $200 billion.
