Requirements
  • An active valid Massachusetts Licensed Practical Nurse license is required.
  • Graduation from a Licensed Practical Nursing program accredited by the National League of Nursing Accreditation Commission is required.
  • An active American Health Association Basic Life Support certification is required.
  • At least two years of experience as a Licensed Practical Nurse in an acute care, rehabilitation, or home care setting is required.
  • Staff must be vaccinated against influenza as a condition of employment.
Responsibilities
  • Perform nursing visits to assigned patients as directed by the Registered Nurse Case Manager or Clinical Manager.
  • Provide hospice nursing care to patients and families using current principles and symptom management as directed by the Registered Nurse.
  • Observe, record, and report significant changes in patients’ healthcare status to the Medical Doctor and Registered Nurse Case Manager.
  • Participate in health teaching required by patients and families.
  • Respect and support patients’ and families’ cultural beliefs and values, individual needs, health goals, and treatment preferences.
  • Communicate with patients, caregivers, and members of the interdisciplinary healthcare team using interpersonal and therapeutic communication skills.
  • Attend regular interdisciplinary team meetings.
  • Practice confidentiality principles established by the agency and federal Health Insurance Portability and Accountability Act guidelines.
