Requirements
  • Must possess a valid Licensed Practical Nurse license issued by the State School of Nursing.
  • Must possess, at a minimum, a high school diploma or a GED.
  • Must read, write, speak, and understand English.
  • Must provide annual verification of a negative tuberculosis skin test.
  • Must provide proof of full COVID-19 vaccination, with exceptions for medical and religious accommodations.
  • Must move intermittently throughout the workday.
  • Must cope with the mental and emotional stress of the position.
  • Must be able to see and hear, or use prosthetics that enable these senses to function adequately.
  • Must meet the facility's general health requirements, including a medical and physical examination.
  • Must be able to work with ill, disabled, elderly, emotionally upset, and occasionally agitated residents.
  • Must push, pull, move, and/or lift at least 25 pounds to a minimum height of 5 feet and carry or move that weight at least 50 feet.
  • May be required to assist with resident evacuation during emergency situations.
  • Must comply with applicable federal, state, and local health and regulatory standards.
  • Must adhere to facility policies, procedures, attendance policies, and employment terms.
Responsibilities
  • Provide care, treatment, and medication administration to assigned residents.
  • Continuously monitor and assess residents' conditions and report in accordance with care plans, medical practices, applicable laws, regulations, and company policies.
  • Provide care to assigned residents based on plans of care.
  • Assist with or administer medications according to the Five Rights of Medication Administration.
  • Reorder and receive medications.
  • Provide personal care and activities of daily living assistance.
  • Supervise care staff.
  • Assist with oversight of ancillary care staff when the Wellness Director or Assistant Wellness Director is absent.
  • Maintain shift-to-shift reports.
  • Provide emergency assistance to residents and complete appropriate follow-up.
  • Obtain and transcribe physician orders.
  • Maintain case management documentation.
