Requirements
  • Current Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of Texas.
  • Provider enrollment with Medicare and Texas state Medicaid
  • Preferred, Minimum of 1 years of clinical experience as an advanced practice provider in a field-based clinical role, preferably in primary care, geriatrics, or a similar field.
  • Preferred but not required, experience with Medicare Advantage programs and familiarity with CMS guidelines and regulations is a plus.
  • Strong interpersonal and communication skills to effectively engage with patients, families, and a diverse healthcare team.
  • Ability to work independently, prioritize tasks, and manage time effectively.
  • Ability to drive to conduct in-home visits
  • Comfortable conducting in-home visits and navigating various home environments.
  • Proficient computer skills for electronic health record documentation.
Responsibilities
  • Conduct comprehensive in-home assessments for complex Medicare patients, including a thorough review of medical history, medication reconciliation, physical examination, and various health assessments (Social determinants of health, mental health, frailty, etc.)
  • Identify and document healthcare needs, potential gaps in care, and risk factors through the assessment process.
  • Develop personalized care interventions based on identified healthcare needs and coordinate with the patient's primary care provider and specialists as necessary.
  • Educate patients and their caregivers on their healthcare conditions, medications, and self-management strategies.
  • Collaborate closely with a multidisciplinary team, including nurse care managers, social workers, pharmacists, community health workers, primary care providers, and specialty providers, to ensure comprehensive care coordination.
  • Provide support and guidance to patients and their families to promote better health outcomes and quality of life.
  • Maintain accurate and up-to-date documentation of assessments, clinical encounters, quality measures, and patient interactions in accordance with Medicare and Medicaid regulatory standards.

Medicare Shared Savings Program: Belong Health and SVMC. * April 9th, 2026 With the expiration of OneCare Vermont at the end of 2025, Southwestern Vermont Medical Center (SVMC) transitioned to a new partnership with Belong Health to participate in the Centers for Medicare and Medicaid Services Medicare Shared Savings Program (MSSP). Through MSSP, Medicare brings together providers in Accountable Care Organizations to offer eligible patients additional coordinated care and services to help them stay as healthy as possible. Below are some frequently asked questions about this partnership and its benefits to its patients. What is an ACO? An ACO, or "accountable care organization," is a team of doctors, hospitals and other healthcare providers who work together to coordinate care for patients. ACOs have agreed with Medicare to be accountable for care quality and cost. They were created to help patients have a better care experience. Is an ACO model new to SVMC? No, SVMC previously participated in an ACO through OneCare Vermont. With the closure of that program, SVMC partnered with Belong Health's ACO. All SVMC providers, practice sites and departments are part of the ACO. How will you benefit from SVMC's participation in an ACO? Southwestern Vermont Medical Center will help you get the right care at the right time. Healthcare can be confusing. Southwestern Vermont Medical Center has a care team who will work with you on your care. Its team may: * Help you know what care you need to have next. * Help you set up appointments. * Help increase access to healthcare providers, both in-person and online. * Make sure you only have the tests and appointments you need. * Connect you with local services or support you may need. * Make sure your medications are safe to take together. * Spot health problems before they become serious. * Answer any questions you have about your care. What changes to my health care should I expect to see as a member of the ACO? In addition to your regular health care providers, you may have access to care coordination services. Its care team will work with you to manage your health conditions and get the services you need. Team members may connect you with health care appointments and local resources as well. They can also help you understand and use your benefits. If I join the ACO, will I still be able to choose my health care providers, including specialists? Yes. You will still be able to see your primary care provider and other providers of your choice, including specialists. Joining the ACO will not restrict your physician network. The Medicare Shared Savings Program does not replace your Medicare or in any way change your Medicare benefits. You can choose or change your main doctor any time. Will joining the ACO cost me anything? There is no cost to you, your Medicare benefits will not change, and you can visit any health care provider or hospital that accepts Medicare. If I do not join the ACO, will I still be able to see my SVMC primary care provider? Yes. You will still be able to see your primary care provider, whether or not you join the ACO. What if I join the ACO and decide later that I do not want to participate anymore? What is the Medicare Shared Savings Program?
