Requirements
  • Graduate from an accredited school of nursing.
  • Hold an active, unrestricted Registered Nurse license in Massachusetts.
  • Have at least one year of clinical Registered Nurse experience with complex medical, behavioral, and social co-morbidities.
  • Be able to conduct assessments in person and through telehealth.
  • Be able to work on interdisciplinary teams.
  • Have skill in screening social determinants of health.
  • Have communication and interviewing skills.
  • Have problem-solving skills and adaptability.
  • Have knowledge of, or willingness to learn, regulatory requirements.
  • Possess a valid driver's license.
  • Attest to having no disqualifiers under the Driver Safety Policy.
  • Possess and provide proof of the minimum state-required auto insurance.
  • Have reliable transportation.
  • Have reliable home internet.
Responsibilities
  • Conduct home visits for onboarding and regulatory assessments.
  • Complete Health Risk Assessments and Personal Care Attendant assessments.
  • Submit Minimum Data Set assessments.
  • Provide education on the NaviCare case management program.
  • Conduct telephonic assessments when appropriate.
  • Collaborate with the Care Team.
  • Complete Long-Term Services and Supports evaluations and collaborate with Utilization Management.
  • Conduct in-home assessments using motivational and culturally sensitive interviewing.
  • Perform medication reconciliation.
  • Complete state-required assessment tools in accordance with the contract.
  • Conduct functional assessments for Long-Term Services and Supports programs.
  • Participate in training and audits.
  • Complete telephonic and virtual assessments.
  • Maintain program and policy knowledge to educate members.
  • Support Healthcare Effectiveness Data and Information Set, Medicare 5-Star, and other initiatives.
Desired Qualifications
  • A Bachelor's or advanced degree in nursing or a health care-related field.
  • Certification in Case Management.
  • Experience in home health, Outcome and Assessment Information Set or Minimum Data Set, Medicare or Medicaid, and face-to-face member interactions.
