Requirements
  • Graduation from an approved certification, program, accredited school, undergraduate school, graduate school, or postgraduate education sufficient to reflect the ability to perform the job.
  • A valid, unencumbered verification or license to practice as described in the state where services are provided.
  • Professional experience as required by state regulations to perform the job.
  • Successful completion of assigned screening and competency testing with a proficiency score of 80% or greater.
  • Reliable transportation.
  • A copy of current tuberculosis results or a chest X-ray.
  • A copy of current cardiopulmonary resuscitation certification according to agency policy.
  • Flexibility, organization, and the ability to exercise independent and sound judgment to perform the job duties and responsibilities.
  • The ability to sit, stand, stop, bend, kneel, walk, balance, crouch, crawl, push, pull, reach with the hands and arms, and perform other physical activities required by the job duties.
  • Vision and hearing capabilities within normal limits, with or without aid.
  • The ability to frequently lift or move up to 15 pounds and occasionally lift or move 100 pounds or more if applicable.
  • The ability to perform the anticipated level of physical assistance required to provide safe client care.
Responsibilities
  • Provide skilled nursing as ordered on the plan of treatment and nursing care plan.
  • Provide skilled nursing care during each shift or visit in accordance with physician orders.
  • Identify new problems, changes, and resolutions and follow through by notifying the director or supervisor, notifying the physician to obtain necessary intervention orders according to state regulations, and documenting the information in clinical notes, supplemental physician orders, and the nursing care plan.
  • Observe aseptic technique as required.
  • Perform specific treatments and administer medications according to the care plan.
  • Function as a team member under the direction of the registered nurse.
  • Work shifts according to the plan of treatment.
  • Communicate skilled observations and significant changes in patient status to the Director of Clinical Services or Clinical Manager.
  • Request modifications to orders or additional orders based on the patient’s clinical status and skilled nursing judgment.
  • Document all physician collaboration in the medical record.
  • Demonstrate and document appropriate utilization of all available services.
  • Participate in other agency-mandated activities, such as in-services, as requested by the Agency Director.
  • Adhere to agency policies, procedures, and handbook requirements.
  • Complete all clinical documentation accurately and submit it promptly according to agency policies and guidelines.
  • Maintain a positive and professional manner in the company and community.
  • Adhere to the dress code with a clean and neat professional appearance.
  • Report on time and as scheduled to complete work within the designated time.
  • Notify the supervisor immediately when the anticipated level of physical assistance exceeds what can safely be provided.
  • Travel frequently to complete home health visits.
