Requirements
  • 6+ months of experience working with or around shelter services
  • High school diploma or GED
  • Communication skills
  • Organization skills
  • Time management skills
  • Valid Colorado Driver’s license with a clean driving record
  • Bilingual Spanish preferred
  • Age 21+ preferred
Responsibilities
  • Monitors activities of daily living for clients and provides training as needed
  • Documents intervention and program activity and maintains accurate and timely clinical records according to Community Reach Center’s policies and procedures
  • Assists consumers with medication administration as directed by Colorado regulations, facility policies and procedures, and physician orders
  • Coordinates care with consumer, families, center staff, and/or external facilities to ensure continuation of care
  • Attends and participates in group sessions, in-services education and training opportunities, meetings, supervisory sessions, review of clinical issues, and special events
  • Acts as a liaison and partners with consumers in the use of basic community resources, referrals, linage, and advocacy
  • Provides therapeutic crisis intervention and emergency services as required
  • Responsible for aiding in house laundry and clean-up of the facility as needed
  • Transports consumers to appointments as needed
  • Manages the milieu and ensure a safe therapeutic environment
  • Complete admission check-in’s to units, provide regular safety checks on units, and monitor clients regularly
  • Maintains confidentiality of consumer records
  • Conduct rounds to ensure safety and compliance of youth in shelter program
