Aledade is pleased to announce a unique internship opportunity that blends population health based special projects, quality improvement methodology, and experience in a clinical setting. This 10 week, 20 to 25 hours/week opportunity can be virtual or hybrid. Under the guidance of an Aledade Medical Director, the physician/clinician lead and/or the practice manager, the intern will have the opportunity to learn about population health principles and observe the characteristics and qualities of a value based primary care practice. The ideal intern may be in undergraduate or postgraduate studies in medicine, pharmacy, public health, behavioral health, nursing or healthcare administration. The ideal intern will be service-minded and interested in data, quality improvement, cost of care, population health and health disparities.
We are flexible with respect to geographic location in North Carolina, and the ideal candidate should be comfortable working remotely. Local applicants interested in more clinical experiences are encouraged to spend at least several sessions on site, depending on COVID guidance at that time and practice preference.
Duties and Responsibilities - Support Population Health initiatives, including:
- Daily huddle workflows for pre-visit planning
- Care gap closure
- Outreach to patients on Aledade worklists (e.g., Wellness, Transitions of Care, Emergency Department follow-up, Care Gap closure, Uncontrolled Blood Pressure, Uncontrolled Diabetes and medication adherence)
Duties and Responsibilities - Develop and implement a special project. Examples:
- Optimizing Telehealth - Develop, assess and assist patients with technology literacy challenges with the goal of optimizing their access to the practice via telehealth platforms
- Improving BP control in underserved populations by helping the practice implement tests of change and measure impact
- Collaborate with the practice’s physician/clinical leadership on protocols for follow-up of positive screenings - example patients with falls risk and curating community based resources
- Studying the root causes of high utilization patterns (frequent ED visits or specialists), developing a test of change, measuring the outcome of the change
- Develop and implement at least 1 campaign to improve a target quality metric (increase HPV vaccination, colon cancer screening, mammography, A1C)
- Engage with Aledade Field Team members to assure accuracy and timeliness of projects and other engagement activities
Requirements:
- Currently enrolled in undergraduate or graduate program
- Excellent attention to detail
- Strong written and verbal communication skills
- Passionate about healthcare and interested in making a positive impact in the field
- Reliable, detail-oriented, and self-motivated to produce work that meets high standards
- Flexible and customer oriented
- Comfortable working in a fast-paced, dynamic environment while maintaining focus on key goals
Preferred:
- Knowledge of Google Suite programs
- Transportation: Valid driver’s license and reliable transportation if traveling to a practice
- Residence in North Carolina. Many partner practices will be located (but not limited to) the following areas: The Triangle, Asheville, Charlotte & Wilmington
