Requirements
  • A high school diploma or GED is required.
  • OASIS and coding certification are required.
  • A minimum of two years of home care experience or experience completing OASIS assessments in E or E1 is required.
  • Strong knowledge of healthcare regulations, accreditation standards, and quality improvement methodologies is required.
  • A background in nursing, healthcare administration, health information management, physical therapy, occupational therapy, speech-language pathology, or MDS is accepted.
  • Strong attention to detail, analytical skills, and problem-solving skills are required.
  • Excellent written and verbal communication skills, including the ability to provide feedback and training to staff, are required.
  • The ability to collaborate effectively with interdisciplinary teams is required.
  • Proficiency in reading, writing, and speaking English is required.
  • Proficiency with Microsoft Office Suite and Windows-based applications is required.
  • The ability to quickly learn and adapt to new software systems is required.
  • The ability to manage multiple priorities in an organized and self-directed manner is required.
Responsibilities
  • Review patient medical records and documentation for accuracy, completeness, and compliance with internal policies, external regulations, Centers for Medicare & Medicaid Services requirements, OASIS guidance, and state and federal laws.
  • Track and analyze key performance indicators, including patient outcomes, readmission rates, medication errors, and adherence to care plans and OASIS guidance.
  • Ensure compliance with healthcare standards, policies, and regulatory requirements.
  • Provide constructive feedback and training to clinical staff regarding documentation practices, quality standards, and improvement opportunities.
  • Identify trends, gaps, and risks in care delivery and collaborate with leadership to develop and implement corrective action plans.
  • Monitor patient safety concerns, adverse events, and near misses and ensure timely reporting and resolution.
  • Collect, interpret, and present data to leadership, committees, and regulatory bodies to support continuous improvement.
Desired Qualifications
  • An Associate’s degree is preferred.
  • ICD-10 coding experience is a plus.
