- A high school diploma or equivalent is required.
- A valid driver's license and reliable transportation are required.
- No prior experience is required; paid training is provided.
- The role requires supporting adults with intellectual, developmental, physical, and behavioral health needs.
- Provide hands-on support for individuals with medical and light behavioral needs in a 14-bed medically frail group home.
- Assist adults with disabilities with daily living activities, including cooking, cleaning, hygiene, dressing, mobility, and personal care.
- Support individuals with bathing, toileting, medication reminders, meal planning, budgeting, communication, and independent living skills.
- Provide transportation to medical appointments, errands, community outings, and recreational activities.
- Encourage participation in volunteer opportunities, fitness programs, Special Olympics, sporting events, crafts, and other community-based activities.
- Implement positive behavior reinforcement strategies and use de-escalation techniques when needed.
- Assist with goal planning, progress tracking, socialization, and skill development to support individualized care plans.
- Collaborate with an interdisciplinary team including nurses, behaviorists, nutritionists, and other clinical professionals to provide comprehensive care.
- Maintain accurate electronic and physical documentation of daily case notes, services provided, progress, and activities.
